Zusammenfassung
Von den jährlich etwa 700 Brandtoten in Deutschland versterben die allermeisten nicht an den Brandwunden, sondern an der Inhalation toxischer Gase während des Verbrennungsvorgangs. Generell ist die brandbedingte Rauchgasinhalation eine komplexe Mischintoxikation. Die genaue Kenntnis der Pathophysiologie und Symptomatik von Intoxikationen sowie der Unfallhergang und die Brandmodalität sind für die präklinische Diagnosebildung wegweisend. Die Basismaßnahmen bestehen aus einer obligaten Sauerstoffgabe und Sicherung der Vitalfunktionen mit Schwerpunkt auf dem Atemwegsmanagement. Die Einleitung einer spezifischen Therapie der führenden Rauchgasintoxikation richtet sich nach der regionalen rettungsdienstlichen Logistik. Eine qualifizierte präklinische Weichenstellung bezüglich Diagnostik und Therapie ist für das Outcome des rauchgasintoxikierten Patienten letztlich entscheidend.
Abstract
In Germany, almost 700 deaths per year result from burn injuries. Most fatalities from fires are not due to burns, but are a result of inhalation of toxic gases produced during combustion. Fire produces a complex environment of smoke and toxic gases. The knowledge of the pathophysiology and symptoms of intoxications and modalities of burn incidents are seminal for preclinical trauma evaluation. The basic treatment includes the obligatory application of oxygen and adequate airway management as well as stabilization of vital functions. The initiation of a specific therapy for acute smoke inhalation injury depends on the structure of the regional emergency medical service. Qualified diagnostics and therapy are essential to improve patients’ outcome in the setting of preclinical emergency treatment on scene.
