Zusammenfassung
Akute Aortendissektionen vom Typ A nach Stanford stellen seltene, lebensbedrohliche Krankheitsbilder dar, die eine umgehende chirurgische Intervention erfordern. In der Prähospitalphase kann die Diagnose leicht als akuter Myokardinfarkt fehl gedeutet werden, insbesondere – wie in unserem Fall dargestellt – bei Patienten ohne charakteristische Schmerzsymptomatik. In Anbetracht der erschwerten Diagnostik sollte vor dem leichtfertigen Einsatz einer präklinischen Lysetherapie, wie sie bei Patienten mit akutem Myokardinfarkt indiziert ist, gewarnt werden.
Abstract
Acute dissections of the aorta classified as Stanford type A represent rare, life threatening events, which require immediate surgical intervention. The disease may be misdiagnosed preclinical as acute myocardial infarction, especially in patients with a lack of characteristic pain sensations. Due to limited diagnostic verification, physicians should be careful of using preclinical lyses therapy as indicated in patients with acute myocardial infarction.
