Zusammenfassung
Hintergrund
Die Erfassung patient*innenberichteter Gesundheitsmerkmale, sogenannter „patient-reported outcomes“ (PRO), ist in der klinisch-epidemiologischen Forschung fest etabliert, bisher aber nur zum Teil in der onkologischen Routineversorgung angekommen. Der vorliegende Beitrag soll einen Überblick über den möglichen Nutzen und die aktuelle Nutzung von PRO in der onkologischen Regelversorgung in Deutschland geben.
Methoden
Wesentliche Begriffe werden definiert, gefolgt von einem kurzen Überblick über aktuelle Anwendungsfelder und Hürden bei der Anwendung. Dann werden Initiativen unter Beteiligung der Deutschen Krebsgesellschaft (DKG) vorgestellt und Überlegungen zur zukünftigen Nutzung von PRO angestellt.
Ergebnisse
Die Auswahl der zu nutzenden PRO hängt vom konkreten Anwendungsfall und insbesondere dem Ziel der Erhebung ab. Wir unterscheiden grob zwei Anwendungsfelder: die individuelle Betreuung der Betroffenen und die Qualitätssicherung bzw. -entwicklung. Neben der inhaltlichen Entscheidung für den Einsatz generischer oder erkrankungsspezifischer Fragebogen ist die Wahl des Modus (vor allem papierbasiert oder online) wesentlich für die konkrete Umsetzung. Einzelne Initiativen setzen PRO bereits in der Routine ein, unter ihnen auch solche der DKG. Erschwert wird eine umfangreichere Nutzung derzeit noch durch die Kosten, den Informationsbedarf aufseiten der Nutzenden, technische Schwierigkeiten und Evidenzlücken.
Schlussfolgerung
Die Autor*innen setzen sich für eine Nutzung von PRO dort ein, wo sie die klinische Versorgung verbessern. Wir machen eine Reihe von Empfehlungen für Forschung und Praxis, unter anderem in Bezug auf weitere Wirksamkeitsuntersuchungen, die Erprobung von PRO zur Versorgungsqualitätsentwicklung und die Entwicklung und den Einsatz von Prädiktionsmodellen.
Abstract
Background
Collection of patient-reported outcomes (PROs) is firmly established in clinical epidemiological research, but has so far only partially been adopted in routine cancer care. This article is intended to provide an overview of the possible benefits and current use of PROs in routine cancer care in Germany.
Methods
Essential terms are defined, followed by a brief overview of current fields of application and hurdles associated with application. We then present initiatives involving the German Cancer Society (DKG) and formulate considerations for future use of PROs.
Results
Selection of the PROs to be used depends on the specific application and goals. We roughly differentiate between two areas of application: individual support for those affected and quality assurance/development. In addition to the content-related decision to use generic or disease-specific questionnaires, the choice of mode (especially paper-based/online) is essential for concrete implementation. Individual initiatives are already using PROs on a routine basis, including those from the DKG. More extensive use is currently made difficult by costs, the need for information on the part of users, technical difficulties, and evidence gaps.
Conclusion
The authors advocate the use of PROs where they improve clinical care. They make a number of recommendations for research and practice, including further effectiveness studies, testing PROs for quality of care development, and the development and use of predictive models.
