Hintergrund
Bildgebende diagnostische Verfahren oder strahlentherapeutische Behandlungen können bei Krebspatienten starke Ängste auslösen. Mitunter kann die psychische Beeinträchtigung so stark sein, dass es zum Abbruch der Maßnahme oder Behandlung kommt.
Ziel der Arbeit
Instrumente zur Identifikation von Risikofaktoren, kommunikative Strategien für den Umgang mit Ängsten sowie präventive und interventive Maßnahmen zur Reduktion von starken Ängsten werden radiologisch tätigen Behandlungsteams vorgestellt.
Methode
Fallbeispiele veranschaulichen die Wissensinhalte und deren praktische Umsetzung.
Ergebnisse
Das strukturierte Gespräch zwischen Behandlern und Patient ist essenziell für den Umgang mit Ängsten im radiologischen Kontext. Screeninginstrumente erleichtern die Beurteilung von Angstsymptomen. Der Einbezug von Psychoonkologen kann hilfreich sein, um die Patienteninformation zu vertiefen (Psychoedukation) und wirksame Interventionen zum Einsatz zu bringen (Entspannungs- und Imaginationsverfahren). Bei Bedarf kann eine Mitbehandlung durch Psychopharmaka erfolgen.
Diskussion
Da Ängste interindividuell stark variieren, können keine allgemeinen Behandlungsansätze empfohlen werden. Es steht aber eine Vielzahl verschiedener Interventionsmöglichkeiten zur Verfügung. Der professionelle Umgang mit Ängsten sollte immer individuell mit dem Patienten abgestimmt werden und jeweils geeignete Methoden berücksichtigen.
Abstract
Background
Diagnostic imaging procedures and radiotherapy can cause severe anxiety in cancer patients. High distress levels can even lead to discontinuation of treatment.
Objective
This article provides an overview of instruments for the identification of risk factors for anxiety, communicative strategies for dealing with anxiety as well as preventive and interventional measures to reduce severe anxiety and to aid healthcare professionals working in radio-oncology.
Method
Case studies are used to illustrate the presented knowledge and the practical implementation.
Results
A structured communication between clinicians and patients is essential for dealing with anxiety in the context of radiology. Screening instruments can facilitate the assessment of anxiety symptoms. The integration of psycho-oncologists can be helpful to enhance patient information (psychoeducation) and to provide effective interventions (e.g. relaxation methods and guided imagination techniques). If necessary, a psychopharmacological treatment can be provided.
Discussion
As anxiety can vary significantly between individuals, no general treatment approaches can be recommended; however, there are a variety of different interventional options. A professional manner of dealing with fears and anxiety should always be agreed with the patient and should consider individual-specific approaches.
