Zusammenfassung
Durchfall ist ein quälendes Symptom, das die Lebensqualität bei Patienten unter palliativer Behandlung einschränkt und mit einer hohen Morbidität und Mortalität assoziiert ist. Bei Patienten mit AIDS handelt sich um ein häufigeres Problem als bei anderen Entitäten wie Krebserkrankungen. Für die symptomatische Behandlung von Diarrhö wird Loperamid als Medikation der ersten Wahl betrachtet. Diese Literaturübersicht untersucht die Effektivität von Loperamid in der symptomatischen Behandlung von Diarrhö in der Palliativmedizin.
Zwei Datenbanken (Embase und Medline) wurden bis Juni 2012 durchsucht. Von 286 identifizierten Studien erfüllten 7 die Einschlusskriterien (1 Kohorten- und 6 experimentelle Studien), in denen Loperamid (allein oder in Kombination) getestet wurde.
Es gibt wenige aussagekräftige Studien, die die Effektivität von Loperamid bei der Diarrhöbehandlung untersucht haben, obwohl es sich bei der Diarrhö um ein belastendes Problem handelt. Zwei Studien zufolge ist Loperamid Placebo überlegen. Im Vergleich mit Octreotid sind die Ergebnisse widersprüchlich. In Kombination mit Acetorphan wirkt Loperamid besser als Acetorphan allein, aber in Kombination mit Diphenoxylat hat es eine schlechtere Wirkung als Octreotid. Die gefundenen Studien zeigten methodische Probleme. Eine klare Empfehlung zur Gabe von Loperamid kann daher aus diesen Arbeiten nicht abgeleitet werden.
Abstract
Diarrhea is a distressing symptom which limits the quality of life in patients receiving palliative care and is associated with high morbidity and mortality. In patients with AIDS, it is a more common problem than for other entities (e.g., cancer). Loperamide is considered the first choice medication for the symptomatic treatment of diarrhea. This literature review examines the efficacy of loperamide in the symptomatic treatment of diarrhea in palliative care. Two databases (Medline and Embase) were searched through June 2012. A total of 286 studies were identified, but only 7 met the inclusion criteria (1 cohort and 6 experimental studies) in which loperamide (alone or in combination) was tested. There is a lack of significant studies which investigate the efficacy of loperamide in the symptomatic treatment of diarrhea. Two trials indicated superiority of loperamide over placebo. In comparison with octreotide, the results were contradictory. The combination of acetorphan with loperamide was more effective than acetorphan alone, but the combination of loperamide with diphenoxylate was inferior to octreotide. The identified studies revealed methodical problems. A definite recommendation for administration of loperamide can, therefore, not be derived from this work.
