Zusammenfassung
Hintergrund
Immunogenität und autologe zelluläre Besiedlung sind die wesentlichen Determinanten des In-vivo-Schicksals biologischer kardiovaskulärer Implantate. Eine Dezellularisierung verhindert zwar eine relevante Immunantwort des Empfängers, allerdings erfordern die aktuellen Protokolle noch weitere Verbesserung, um gleichzeitig Azellularität, Präservierung der extrazellulären Matrix (EZM) und Biofunktionalität garantieren zu können.
Ziel der Arbeit
Die aktuelle Studie diente der Evaluation eines detergenzienfreien, nichtproteolytischen, aktinabbauenden Verfahrens (BIO) zur Dezellularisierung von Herzklappen- und Gefäßprothesen. Ein besonderer Fokus lag auf der Biofunktionalität der Implantate.
Material und Methoden
Die Dezellularisierung von Schweineaortenklappengewebe („porcine aortic valve“, pAV; n = 106) und Rattenaortenkonduits („rat aortic conduit“, rAoC; n = 89) wurde durch biologische Agenzien (Gruppe BIO: Latrunculin B, Salzlösungen mit hoher Ionenstärke, DNase I) und Detergenzien (Kontrollgruppe DET: Natriumdodecylsulfat, Desoxycholat) erzielt. Strukturelle und mechanische Aspekte sowie die Zytokompatibilität der Prothesen wurden in vitro analysiert, während ihre Biofunktionalität über 8 Wochen in einem Rattenmodell der Implantation in die systemische Zirkulation untersucht wurde.
Ergebnisse
Im histologischen und im immunhistologischen Nachweis, in der Western-Blotting-Methode sowie in der Raman-Spektroskopie zeigten sich BIO-dezellularisierte Prothesen frei von zellulären Komponenten, während die Struktur der EZM und die mechanischen Eigenschaften erhalten waren. Die Architektur der BIO-rAoC ermöglicht im Vergleich zu DET-dezellularisierten Grafts eine verbesserte In-vivo-Biofunktionalisierung durch Fibronektinbeschichtung. Dies stellte sich sowohl durch eine beschleunigte autolog-zelluläre Besiedlung der Media (p < 0,001) als auch durch eine verminderte Intimahyperplasie (p < 0,001) dar. Weiterhin spielten inflammatorische Prozesse keine Rolle in den an den Explantaten erhobenen Befunden.
Schlussfolgerung
Das biologisch basierte, detergenzienfreie, nichtproteolytische Verfahren balanciert effektive Dezellularisierung und Erhalt der EZM in kardiovaskulären Prothesen und liefert außerdem eine optimierte Biofunktionalisierbarkeit. Darüber hinaus könnte das aktinabbauende Verfahren eine vielversprechende Methode zur Herstellung nichtimmunogener, azellulärer Gerüste aus anderen muskulären Geweben, wie z. B. Myokard oder Darmwand, sein.
Abstract
Background
The in vivo fate of biological cardiovascular implants is predominantly influenced by the immunogenicity and autologous cellular population of the graft. Decellularization inhibits relevant immune responses against the prostheses; however, current protocols still need to be improved to guarantee acellularity, extracellular matrix preservation and biofunctionality to the same extent.
Objective
In this study a detergent-free, non-proteolytic, actin-disassembling regimen (BIO) for the decellularization of heart valves and vessel grafts, particularly focusing on the biofunctionality was evaluated.
Material and methods
Decellularization of porcine aortic valve tissue (pAV, n = 106) and rat aortic conduits (rAoC, n = 89) was achieved by biological agents (group BIO: latrunculin B, high ionic strength solutions and DNase I) or detergents as controls (group DET: sodium dodecyl sulfate and deoxycholate). Structural and mechanical aspects as well as the cytocompatibility were analyzed in vitro, while the biofunctionality was examined over 8 weeks in a rat model of systemic implantation.
Results
Prostheses decellularized in group BIO were free from cellular components, as shown by histological and immunohistological detection as well as in Western blotting and Raman spectroscopy, while the extracellular matrix structure and the mechanical properties were preserved. The architecture of BIO-rAoC enables improved in vivo biofunctionalization using fibronectin with significantly increased speed and amount of autologous cellular repopulation of the media (p < 0.001) and decreased formation of hyperplastic intima (p < 0.001) as compared to fibronectin-coated grafts decellularized in the DET group. Moreover, inflammatory processes did not play a role in any of the explanted grafts.
Conclusion
The biological, detergent-free, non-proteolytic regimen balances effective decellularization and extracellular matrix preservation in cardiovascular grafts and provides optimized biofunctionalization. Moreover, the actin-disassembling regimen may be a promising approach for bioengineering of non-immunogenic, acellular scaffolds from other muscular tissues, such as the myocardium or intestinal wall.
