Maßnahmen zur Konditionierung eines postmortalen potenziellen Spenders vor der Multiorganentnahme

Intensive care therapy after brain death demands special attentions from the physican in charge, because only in the case of optimal homeostase, is it possible for the organs to fully perform their function after explantation. In addition to maintaining sufficient blood circulation and oxygenation, it is important to detect developing hypernatriemia, which could be a risk for the proper function of the organs. We could show that it is possible to treat developing hypernatriemia and hyponatriuria with Pitressin (Vasopressin) and that serum sodium concentration was normal in a short time after s.c. as well as i.v. treatment. Therefore after decerebration and developing hypernatriemia we recommend a dose of Pitressin.

Zusammenfassung

Die Intensivtherapie nach eingetretenem Hirntod verlangt vom betreuenden Arzt besondere Aufmerksamkeit, denn nur bei optimaler Homöostase können die Organe nach der Entnahme ihre Funktion voll erfüllen. Neben der Aufrechterhaltung eines suffizienten Kreislaufs und einer ausreichenden Oxygenation ist es wichtig, eine sich entwickelnde Hypernatriämie durch ADH-Verlust zu erkennen, da sie eine große Gefahr für das Transplantat darstellt. Wir konnten zeigen, dass eine Hypernatriämie und Hyponatriurie gut durch die Gabe von Pitressin (Vasopressin) zu behandeln ist und sich der Serumnatriumspiegel sowohl nach s.c.-Gabe als auch nach i.v.-Gabe innerhalb kurzer Zeit normalisiert.¶   Wir empfehlen deshalb nach eingetretener Dezerebration und sich entwickelnder Hypernatriämie und Hyponatriurie die Gabe von Pitressin.
