Abstract
We report on a 28-year-old previously healthy female patient who developed a rightsided hemiparesis and hemisensory deficit, a left-sided homonymous hemianopia, as well as a psychomotor slowing within a period of 6 weeks. MR imaging proved multiple contrast-enhancing lesions and lumbar puncture revealed intrathecal immunoglobulin G synthesis. Despite twofold high-dose i.v. corticosteroid therapy, no improvement of symptoms was present. Probably owing to a pre-existing prothrombin-Amutation, thromboses of the transverse sinus and the sigmoid sinus occurred during the steroid therapy and led to the initiation of anti-coagulant treatment. When the patient developed a tetraparesis and bilateral amaurosis, and follow-up MR imaging showed an increase of contrastenhancing lesions, treatment with i.v. mitoxantrone was initiated. Because of the inevitable anticoagulant treatment, no brain biopsy was undertaken. Mitoxantrone (12 mg/m2 BS) was administered three times at 4-week intervals, and intravenous immunoglobulins (10 g/5 days) were administered additionally. Hereby, the patient had a substantial clinical improvement, and MRI imaging showed a considerable lesion decrease. Due to the positive response to mitoxantrone treatment, the ineffectiveness of highdose steroid therapy and the cerebrospinal fluid findings, we assume that the patient had Marburg type multiple sclerosis, an acute variant of multiple sclerosis that may only improve with intense immunosuppression.
Zusammenfassung
Wir berichten über eine 28-jährige Patientin, die innerhalb von 6 Wochen aus völliger Gesundheit heraus eine Hemiparese, -hypästhesie rechts, Hemianopsie nach links und eine psychomotorische Verlangsamung bei kernspintomographisch nachweisbaren kontrastmittelaufnehmenden Herden und intrathekaler IgG-Synthese entwickelte. Trotz einer zweimalig durchgeführten Cortisonstoßtherapie kam es zu keiner Besserung der Symptomatik. Unter Steroidtherapie bei heterozygoter Prothrombin-A-Mutation kam es zu einer Sinus transversus- und Sinus sigmoideus-Thrombose links mit konsekutiv notwendiger oraler Antikoagulation. Nach weiterer Zunahme der kernspintomographischen Herde und Auftreten einer Tetraparese und Amaurose beidseits erfolgte die Gabe von Mitoxantron. Aufgrund der oralen Antikoagulation wurde auf eine bioptische Klärung der Herde verzichtet. Unter Induktionsbehandlung mit der 3-maligen Gabe von Mitoxantron (12 mg/m2 KOF) im Abstand von 4 Wochen und im weiteren Verlauf zusätzlicher intravenöser Gabe von 7s-Immunglobulin (10 g/5 Tage), kam es innerhalb von 12 Wochen zu einer deutlichen Besserung der Klinik und des kernspintomographischen Befundes. Aufgrund des guten Ansprechens auf Mitoxantron, der Ineffektivität von Cortison und des Liquorbefundes gehen wir von einer Multiplen Sklerose vom Typ Marburg aus, deren Prognose sich erst seit Einsatz von Mitoxantron verbessert hat.
