Pathophysiology of burn trauma

Summary

This article reviews the pathophysiological reactions of local burn injury and the resulting systemic response of the human body to severe thermal lesions. Referring to immunological reactions on the cellular level, the genesis of systemic inflammatory response syndrome (SIRS) and the accompanying sepsis is exposed in the treatment of severely burned trauma patients. In addition, the two modern theories (two hit model, model of posttraumatic immunsuppression) in the formation of multiple organ failure are discussed. Finally, the pathophysiological response of different vital organs as a result of severe burn trauma and the providing of modern options for treatment are explained, with focus on the intraabdominal compartment syndrome.

Zusammenfassung

Der Artikel gibt einen Überblick über die lokale und daraus entstehende systemische pathophysiologische Reaktion des Körpers auf eine großflächige thermische Schädigung. Aus den immunologischen Vorgängen auf zellulärer Ebene wird die Entstehung des systemic inflammatory response syndrome (SIRS) und der sich daraus entwickelnden Sepsis im Rahmen des Verbrennungstraumas bei schwerbrandverletzten Patienten dargelegt. Ebenfalls werden die zwei derzeitigen Theorien (Two Hit Modell, Modell der posttraumatischen Immunsuppression) der Entstehung des Multiorganversagens diskutiert. Abschließend werden die pathophysiologischen Antworten der einzelnen lebenswichtigen Organsysteme auf das schwere Verbrennungstrauma einschließlich der derzeitigen modernen Therapiemöglichkeiten erläutert, wobei hierbei besonders auf das intraabdominelle Kompartmentsyndrom eingegangen wird.
