Zusammenfassung
Hintergrund
Die Pupillotonie ist eine der häufigsten parasympathischen Innervationsstörungen. Entgegen der Lehrmeinung neuerer Lehrbücher ist auch bei der isolierten Pupillotonie die kranielle Bildgebung weiterhin gängige Praxis. Es sollte nachgewiesen werden, dass sie nicht erforderlich ist.
Patienten und Methode
Die Befunde von 33 Patienten mit Pupillotonie wurden retrospektiv ausgewertet. Bei allen Patienten wurde eine komplette ophthalmologische, orthoptische und neurologische Untersuchung durchgeführt. Eine bildgebende Diagnostik des Schädels erfolgte bei allen Patienten mit MRT oder CT. In einigen Fällen wurden serologische Tests vorgenommen.
Ergebnisse
Bei keinem der Patienten zeigte die bildgebende Diagnostik des Schädels einen Befund, der eine zusätzliche Information zu einer möglichen Ursache der Pupillotonie erbrachte.
Schlussfolgerung
Eine kranielle Bildgebung ist bei der isolierten Pupillotonie nicht erforderlich. Aus therapeutischer Sicht ist entsprechend unserer Patientenserie und Literaturangaben eine weiterführende Diagnostik nur bei Patienten über 50 Jahre zum Ausschluss einer Arteriitis temporalis und Lues zu empfehlen.
Abstract
Background
Tonic pupil (TP) is a common disorder of parasympathetic innervation. In contrast to textbook recommendations, cranial imaging is still being performed in most of the patients with TP. The intention of the present study is to show that cranial imaging is of no benefit.
Patients and methods
The medical records of 33 patients with TP were analyzed retrospectively. All patients had undergone a complete ophthalmological, orthoptic, and neurological investigation. Cranial imaging was performed by computed tomography or magnetic resonance imaging. Serology tests were carried out in some of the patients.
Results
Diagnostic imaging provided no additional data revealing the underlying cause of TP.
Conclusions
Cranial imaging in isolated tonic pupil is not helpful. Because of therapeutic implications, diagnostic evaluation can be recommended only in patients older than 50 years to exclude giant cell arteritis and syphilis.
