Zusammenfassung
Anhand der Beobachtung einer Manifestation des Proatlas mit Ausbildung eines zusätzlichen zervikalen Segments bei einem 8‑jährigen Jungen wird das Krankheitsbild diskutiert. In den Röntgenbildern ließen sich 2 Massae laterales proatlantes und Massae laterales atlantes nachweisen. Der Arcus dorsalis proatlantis ist nicht geschlossen, der Arcus dorsalis atlantis erscheint hypoplastisch. Die biofunktionellen und entwicklungsanatomischen Besonderheiten dieser Dysplasie sollten von anderen Manifestationsformen des 4. Okzipitalwirbels abgegrenzt werden. Die von uns als „Tsuang-Göhmann-Malformation“ bezeichnete Fehlbildung ist charakterisiert durch die Etablierung eines zusätzlichen zervikalen Segments oberhalb des Atlas. Die entwicklungsanatomischen Hintergründe mit Verschiebung der Kopf-Hals-Grenze nach kranial werden diskutiert.
Abstract
We present the case of an 8‑year-old boy with proatlas manifestation and additional cervical segmentation. The disease and the malformation will be discussed. Cervical spine radiographs demonstrated two massae laterales proatlantes, and two massae laterales atlantes. The arcus dorsalis proatlantis is not fused and the arcus dorsalis atlantis is hypoplastic. The biomechanical and embryological features of this dysplasia should be differentiated from other malformations of the fourth occipital vertebra. We propose the term “Tsuang–Göhmann malformation,” which is characterized by an additional cervical segment due to the formation of two massae laterales proatlantis over the atlas. The embryological background with displacement of the craniocervical boundary will be discussed.
