Endo-prothèse fémorale personnalisée dans le traitement d’une agénésie fémorale de l’adulte jeune

Summary.

We describe the management of femoral agenesis in a young adult. Following previous treatment, including limb lengthening by the Ilizarov technique, the patient was left with 30 cm of shortening, a hip fused in abduction and external rotation, an ankylosed knee and an ankle which had been surgically fused. We decided to create a mono-segmental stump, based on a mobile hip, in order to accommodate a suitable prosthesis. Replacement arthroplasty of the hip was first undertaken using a femoral component with a long stem which crossed the knee into the upper tibia. This allowed correction of the rotational deformity and fusion of the knee in an appropriate position. Amputation above the ankle was then carried out under the same anaesthetic. A suction prosthesis was attached to the stump. The arthrodesis of the hip was the principle factor in deciding on this procedure rather than a Van Ness operation or further lengthening. The result at 3 years is satisfactory and the patient has good mobility.

Nous rapportons un cas d’agénésie fémorale sévère chez un adulte jeune. Le handicap, très lourd, était en relation avec une triple ankylose hanche-genou-cheville, et une inégalité de plus de 30 cm. En particulier, la hanche était fusionnée en abduction-rotation externe à la suite d’un allongement par la méthode Ilizarov. Dans cette conformation, les possibilités d’améliorer l’appareillage étaient nulles. L’intervention chirurgicale réalisée à combiné une désarthrodèse prothèse de hanche, une fémoralisation du tibia par arthrodèse du genou, et une amputation au-dessus de la cheville, afin de créer un moignon mobile uni-segmentaire. Cette intervention en un temps a été rendue possible par la conception et la fabrication assistées par ordinateur d’une tige fémorale sur mesure longue pontant le genou et dérotant le membre. Par la suite, le moignon a pu être appareillé simplement par une prothèse contact de cuisse. L’arthrodèse de hanche fut l’élément déterminant pour préférer cette technique à une chirurgie d’alignement, un Van Nes ou une nouvelle tentative d’allongement. Le résultat à trois ans nous conforte dans ce choix.
