Revision Total Hip Replacement with Transfemoral Extended Osteotomy

Abstract

Uncemented fluted femoral stems were designed by Heinz Wagner for use in patients with bone stock that is insufficient for proximal metaphyseal fixation. The original description included a particular approach called “Transfemoraler Zugang” that is splitting the femur with a longitudinal osteotomy. Seventy-four revision total hip arthroplasties were performed using this approach with a conical, fluted stem (Restoration T3 – Howmedica Stryker). The mean follow-up was 7.1 years. The clinical results were quite satisfactory. Subsidence in excess of 10 mm was present in 8 % of the cases, in no case progressive. There were no particular complications related to the surgical approach. Femoral reconstruction was achieved in 88 % of the cases. No patient required re-revision of the stem. Transfemoral Extended Osteotomy in association with conical, fluted stems is an effective technique and gives quite satisfactory long-term results.
