Abstract
Purpose
The quality of external post-mortem examinations and medical death certificates has long been criticized and autopsy rates are on the decline. To identify potential errors in external post-mortem examinations and hospital death certificates, this study examined the quality of information provided regarding both aspects at the University Medical Center Hamburg-Eppendorf (UKE).
Methods
Among the 630 patients who died at the UKE during a period of 5 months in 2017, the post-mortem findings and death certificates of 511 were re-examined. Furthermore, for 419 deceased patients where a forensic autopsy was carried out at the UKE from 2008 to 2016, the clinical data were compared with the autopsy results.
Results
For 77 (15%) of the decedents, the second external post-mortem examination produced relevant findings that had not been previously reported on the death certificates. This included information regarding relevant medical interventions (9%), significant disorders (5%), and trauma (2%). Formal errors were rarely found on the death certificates. In 62% the cause of death and underlying disorders stated in the death certificates were in full agreement with the diagnoses identified at autopsy. Nonconformities were identified in 10%.
Conclusion
The quality of external post-mortem examinations and medical death certificates should be improved. It is evident that autopsies are indispensable for verifying the cause of death.

Zweck
Die Qualität von Leichenschau und Todesbescheinigung wird seit langem kritisiert, und die Autopsieraten sinken. Um mögliche Fehler bei Leichenschau und Todesbescheinigung im Krankenhaus zu identifizieren, wurde in dieser Studie die Qualität der Informationen bezüglich beider Aspekte am Universitätsklinikum Hamburg-Eppendorf (UKE) untersucht.
Methoden
Von den 630 Patienten, die 2017 am UKE während eines Zeitraums von 5 Monaten starben, fand bei 511 eine Überprüfung der Leichenschau und Todesbescheinigung statt. Darüber hinaus wurden bei 419 verstorbenen Patienten, die von 2008 bis 2016 am UKE rechtsmedizinisch autopsiert worden waren, die klinischen Angaben mit den Autopsieergebnissen verglichen.
Ergebnisse
Bei 77 (15 %) Verstorbenen lieferte die zweite Leichenschau relevante Befunde, die bisher in der Todesbescheinigungen nicht aufgeführt waren. Dazu gehörten Informationen über relevante medizinische Interventionen (9 %), relevante Erkrankungen (5 %) und Traumata (2 %). Formale Fehler wurden auf den Todesbescheinigungen selten gefunden.
In 62 % der Fälle stimmte die in der Todesbescheinigung angegebene Todesursache und die zugrunde liegenden Erkrankungen mit den in den Autopsien gefundenen Diagnosen überein. Zu 10 % wurde keine Übereinstimmung festgestellt.
Schlussfolgerungen
Die Qualität von Leichenschau und Todesbescheinigung sollte verbessert werden. Es ist offensichtlich, dass Autopsien unersätzlich sind, um die Todesursache zu sichern.
