Zusammenfassung
Periglenoidale Zysten treten bei etwa 2–4 % der Bevölkerung auf, wobei hauptsächlich Männer mittleren Alters betroffen sind. Zystische Formationen in diesem Bereich können mit Schmerzen und Muskelschwäche aufgrund einer Kompressionsneuropathie des N. suprascapularis (SSN) einhergehen. In diesem Beitrag wird der Fall eines 43-jährigen, sportlich aktiven Patienten beschrieben, der sich aufgrund von Schmerzen und diffusem zunehmendem Kraftverlust in der linken Schulter vorstellte. Mittels Magnetresonanztomographie (MRT) wurde eine massive Zyste sowie eine Hyperintensität der Muskulatur als Korrelat eines Ödems aufgrund einer Denervierung diagnostiziert. Da eine konservative Therapie frustran verlief, erfolgte ein arthroskopischer Eingriff unter Zuhilfenahme zweier supraskapulärer Portale. Hierunter konnten die Resektion der Zyste sowie die Dekompression des SSN erfolgreich durchgeführt werden. Die Schulterarthroskopie ist bei entsprechender Erfahrung des Operateurs eine erfolgversprechende Option zur Behandlung periglenoidaler Zysten, da hier gleichzeitig eine Nervendekompression erfolgen kann und auch vorliegende Begleitpathologien wie eine Labrumläsion mit adressiert werden können.
Abstract
Periglenoid cysts occur in about 2–4 % of the population, mainly affecting middle-aged men. Cystic formations in this area may be associated with pain and muscle weakness due to compression neuropathy of the suprascapular nerve (SSN). This article describes the case of a 43-year-old, athletically active patient who presented due to pain and diffuse increasing loss of strength in the left shoulder. Magnetic resonance imaging (MRI) showed a massive cyst as well as hyperintensity of the muscles as a correlate of edema due to denervation. As conservative therapy had failed, arthroscopic surgery was performed using two suprascapular portals. Resection of the cyst and decompression of the SSN were successfully performed. For the experienced surgeon shoulder arthroscopy is a promising option for the treatment of periglenoid cysts, as nerve decompression can be performed at the same time and accompanying pathologies, such as labral lesions, can be addressed as well.
