Zusammenfassung
Hintergrund
Von der Geburt bis zum Wachstumsende durchlaufen die Beinachsen physiologische Veränderungen. Angeborene, idiopathische oder sekundär entstehende Deformitäten der unteren Extremitäten gelten als Präarthrose. Eine sorgfältige klinische Untersuchung steht im Vordergrund. Anhand der etablierten Vorsorgeuntersuchungen können die asymptomatischen Deformitäten von den alterstypisch physiologischen Achsabweichungen bereits frühzeitig unterschieden werden.
Therapie
Es existiert eine Vielzahl an konservativen und operativen Therapiemöglichkeiten. Oft kann auch der Spontanverlauf je nach Deformität und damit einhergehender Einschränkung abgewartet werden. Wichtig ist, den richtigen Interventionszeitpunkt zu erkennen. In der Regel sind dann operative Korrekturen indiziert, hierzu zählen permanente oder temporäre Wachstumslenkungen sowie Umstellungsosteotomien. Ziel ist es, die anatomischen Achsverhältnisse wiederherzustellen und so der Entstehung einer Arthrose vorzubeugen. Eine verpasste Intervention mündet im Erwachsenenalter nicht selten in einem frühzeitigen endoprothetischen Ersatz.
Abstract
Background
From birth to the end of growth, leg axes undergo physiological changes. Congenital, idiopathic or secondary deformities of the lower extremities are considered as a pre-arthrosis. A detailed clinical examination is of primary importance. On the basis of the established preventive examinations, predominantly asymptomatic deformities can be distinguished from age-typical physiological axis deviations at an early stage.
Therapy
There is a variety of conservative and surgical therapy options. In most cases, the spontaneous course of development can be waited for, depending on the deformity and the associated restrictions. It is important to recognize the right time for an intervention. Usually, surgical interventions are necessary, including guided growth by permanent and partial arrest of the growth plate or osteotomies. The aim is to restore the anatomical axial alignment and thus prevent the development of osteoarthritis. A missed intervention often leads to an early endoprosthetic replacement in adulthood.
