Einzeitiger aseptischer Wechsel gelockerter Großzehengrundgelenkendoprothesen

One-stage aseptic revision of loosened metatarsophalangeal prosthesis

Zusammenfassung

Aufgrund fehlender knöcherner Integration der keramischen Moje-Prothese am 1. Metatarsophalangealgelenk (MTP I) waren die mittelfristigen Ergebnisse nach Implantation bei einem Hallux rigidus bisher schlecht. Im Revisionsfall ist die Distraktionsarthrodese mit Beckenkammspanentnahme die Salvageprozedur der Wahl. In dieser prospektiven Fallserie wurden die kurzfristigen Ergebnisse nach Wechsel einer gelockerten Moje-Prothese auf die TOEFIT-PLUS™-Prothese untersucht. Die klinischen und radiologischen Untersuchungen erfolgten an insgesamt 3 MTP-I-Gelenken über einen Zeitraum von 12 Monaten. Der AOFAS-Score und VAS wurden präoperativ, 6 und 12 Monate postoperativ erhoben.

Nach Wechsel einer gelockerten Moje- auf die TOEFIT-PLUS™-Prothese konnten gute bis sehr gute kurzfristige Ergebnisse gezeigt werden. Bei noch ausreichender knöcherner Verankerungsmöglichkeit kann unter Erhalt und Verbesserung des Bewegungsumfanges hierdurch die Distraktionsarthrodese mit entsprechender Beckenkammspanentnahmemorbidität vermieden werden.

Abstract

The high loosening rate of the Moje prosthesis in the treatment of hallux rigidus caused disappointing medium term results. Arthrodesis using an iliac crest bone graft is the standard salvage procedure. We present short-term results after one-stage revision of the metatarsophalangeal joint using the TOEFIT-PLUS™ prosthesis for implant revision. Clinical and radiological examinations were performed on three metatarsophalangeal joints after explantation of the Moje prosthesis and one-stage revision using the TOEFIT-PLUS™ prosthesis for implant revision. The AOFAS score and VAS were checked preoperatively and 6 and 12 months after surgery.

Good to very good short-term results after one-stage revision of the metatarsophalangeal joint using the TOEFIT-PLUS™ prosthesis for implant revision could be demonstrated in this study. One-stage revision of the arthroplasty of the metatarsophalangeal joint can increase the range of motion and avoid arthrodesis entailing iliac crest bone graft morbidity.
