Zusammenfassung
Eine unzureichende postoperative Schmerztherapie kann durch eine neuronale Plastizität zu einem chronischen Schmerzgeschehen und damit einem schlechten Operationsergebnis führen. Die Anwendung eines multimodalen Konzepts unter Einschluss medikamentöser Behandlungsformen, der Regionalanästhesie sowie physikalischer und psychologisch verhaltenstherapeutischer Verfahren ist deshalb zur Prophylaxe in der postoperativen Schmerztherapie sinnvoll. Damit lässt sich nicht nur die Schmerzintensität verringern, sondern es können auch Nebenwirkungen einzelner Maßnahmen reduziert und die Krankenhausverweildauer verkürzt werden. Eine wirksame Schmerztherapie ist von zentraler Bedeutung für die adäquate postoperative Mobilisation als wichtige Voraussetzung für ein gutes funktionelles Ergebnis nach Operationen am Bewegungsapparat. Weiterhin sollte nicht unterschätzt werden, dass die Patientenzufriedenheit als wichtiger Benchmarking-Faktor nach einem stationären Aufenthalt ganz wesentlich von der Qualität einer postoperativen Schmerztherapie beeinflusst sein kann.
Abstract
Under-treatment of acute postoperative pain can lead to chronic pain with neuronal plasticity and result in poor surgical outcomes. A multimodal approach is therefore necessary to reduce postoperative pain by combining various analgesics with a non-pharmacological strategy. The current use of multimodal approaches, even for the management of postoperative pain, can reduce the side effects of pharmaceutical therapy alone as well as reducing the length of hospital stay. Adequate pain control is an important prerequisite for the application of rehabilitation programmes and will thereby influence functional outcome. In addition, patient satisfaction, as a major benchmarking factor after surgical treatment, is significantly influenced by the quality of postoperative pain management
