Zusammenfassung
Die anale Inkontinenz ist häufig Thema in der urogynäkologischen und koloproktologischen Sprechstunde; meist wird der Gynäkologe zum ersten Ansprechpartner Betroffener. Dem Symptom Stuhlinkontinenz können verschiedene Ursachen zugrundeliegen. Ein intakter Beckenboden ist die Voraussetzung für urethrale und anale Kontinenz. Die vaginale Geburt ist eine der Hauptursachen für Läsionen des Beckenbodens, ebenso nervale Schädigungen oder Kolonmotilitätsstörungen. Zunächst ist es wichtig zu erkennen, in welchem Ausmaß Defekte im Sphinkterapparat vorhanden sind. Zur Diagnostik steht neben der gynäkologischen Untersuchung die apparative Diagnostik mittels Endoanalsonographie, dynamischer MRT-Untersuchung und Analmanometrie zur Verfügung. Die Betreuung dieser Patientinnen sollte interdisziplinär mit einem Zentrum erfolgen. Zur Therapie sind viele Verfahren beschrieben, hierbei können oft konservative Maßnahmen wie intensiviertes Beckenbodentraining oder Stuhlregulierung einen ausreichenden Erfolg erzielen. Bei den operativen Therapien sind neben fixierenden Techniken peranale Operationsverfahren etabliert. Wichtige Bestandteile der Prävention sind verändertes geburtshilfliches Management, suffiziente Nachsorge höhergradiger Geburtsverletzungen sowie die Planung des optimalen Geburtsmodus.
Abstract
Anal incontinence is a recurrent component of gynecological and coloproctological consultation and gynecologists are often the first contact person for affected women. The symptom of incontinence may have different causes. A functional pelvic floor is necessary for anal and urethral continence and vaginal birth is one of the main reasons for pelvic floor damage and incontinence. Neural lesions and bowel motility failure can also cause secondary anal incontinence, therefore it is important to identify the extent of the lesions in the anal sphincter muscles. For the diagnostic approach clinical and gynecological examinations with endoanal sonography, defecography and anal manometry yield decisive information. A great variety of methods are available for treatment and even conservative therapy alone will often be successful. Surgical reconstruction is the method of choice for all traumatic lesions. The prevention of stool incontinence with good obstetric management, sufficient follow-up of obstetric damage and development of an optimal birth strategy is becoming part of the gynecological routine. The treatment should be interdisciplinary and in cooperation with a specialized center.
