Zusammenfassung
Hintergrund
Die computertomographische Koronarangiographie (cCTA) hat einen hohen negativ-prädiktiven Wert für den Ausschluss signifikanter Koronarstenosen. Im Fall einer detektierten Stenose ist sie jedoch darin limitiert, die hämodynamische Relevanz zu bestimmen und damit die Therapieindikation zu prüfen.
Fragestellung
Wann und wie wird die cCTA gemäß den aktuellen Leitlinien eingesetzt? Welche funktionellen CT-Techniken gibt es zur Abklärung der hämodynamischen Relevanz einer Stenose?
Material und Methoden
Der Stellenwert der cCTA wird anhand der aktuellen Leitlinien der European Society of Cardiology (ESC) zur Diagnostik des chronischen Koronarsyndroms besprochen. Funktionelle CT-Techniken zur Bestimmung der hämodynamischen Relevanz einer Koronarstenose sind die myokardiale CT-Perfusion (CT‑P) sowie die CT-basierte fraktionelle Flussreserve (CT-FFR).
Ergebnisse
Die cCTA hat in den Leitlinien der ESC eine Klasse-1-Empfehlung für die Diagnostik der koronaren Herzkrankheit (KHK) bei symptomatischen Patienten mit niedriger klinischer Wahrscheinlichkeit für eine KHK. Bei hoher Wahrscheinlichkeit oder einer mittels cCTA detektierten, mindestens mittelgradigen Stenose sollte zunächst eine nichtinvasive funktionelle Bildgebung eingesetzt werden. Sowohl die CT‑P als auch die CT-FFR haben bereits einen hohen Grad an Evidenz.
Schlussfolgerung
Die cCTA hat in den aktuellen Leitlinien einen hohen Stellenwert als zuverlässige Methode zur Abklärung einer KHK. Die funktionellen Verfahren CT‑P und CT-FFR zeigten in zahlreichen klinischen Studien eine hohe diagnostische Genauigkeit für den Nachweis einer koronaren Ischämie. Diese Methoden sind noch nicht fester Bestandteil der aktuellen Leitlinien und werden klinisch noch nicht weit verbreitet eingesetzt.
Abstract
Background
Coronary CT angiography (cCTA) has a high negative predictive value for ruling out significant coronary stenoses. However, it is limited in determining hemodynamic significance of a stenosis and hence to prove the indication for therapy.
Objectives
When and how is cCTA used according to current guidelines? Which functional CT techniques are available to test for hemodynamic significance of coronary stenoses?
Materials and methods
The value of cCTA is explained on the basis of current guidelines by the European Society of Cardiology (ESC) for the diagnosis of chronic coronary syndromes. Functional CT techniques which test for hemodynamic significance of coronary stenoses are myocardial CT-perfusion (CT‑P) and CT-based fractional flow reserve (CT-FFR).
Results
The new ESC guidelines classify cCTA as a class 1 recommendation for diagnosing coronary artery disease in symptomatic patients with low clinical likelihood. If clinical likelihood is high or an at least moderate stenosis is detected with cCTA, noninvasive functional (stress) imaging is preferred. There is a large body of evidence for CT‑P and CT-FFR as functional tests.
Conclusion
In the current guidelines, cCTA is highly recommended for the diagnosis of coronary artery disease. The functional CT techniques CT‑P and CT-FFR have shown high diagnostic accuracy for myocardial ischemia of coronary stenoses in many clinical studies. However, these methods are not part of current guidelines yet and clinical adoption is still low.
