Radiologische Standardverfahren
Neue technische Entwicklungen auf dem Gebiet der endovaskulären Versorgung infrarenaler Aortenaneurysmen (EVAR) führen dazu, dass ein immer größeres Spektrum an unterschiedlichen Aortenpathologien einer minimal-invasiven Therapie zugeführt werden kann.
Leistungsfähigkeit
Die Verbesserung von Einführkathetern hinsichtlich Diameter und Stabilität kombiniert mit Flexibilität sind wichtige Parameter, die insbesondere bei ungünstigen stenosierten iliakalen Zugangswegen entscheidend für die Machbarkeit einer EVAR werden. Einfachheit und Effektivität in der Anwendung führen zu mehr Sicherheit und Erhöhung der Prozedurqualität beim Interventionalisten zugunsten der Behandlungsqualität des Patienten. Die Weiterentwicklung der Aortenstentprothesen mit der Möglichkeit zur Repositionierung, aktiver Positionierbarkeit oder neuer Konzepte der endoluminalen Abdichtung lassen eine verbesserte Behandlung einer starken Angulation oder ungünstiger Verankerungszone zu.
Empfehlung für die Praxis
Dem Interventionalisten kommt eine immer größere Verantwortung zu, das verfügbare Prothesenmaterial kritisch zu prüfen und individuell für jeden Patienten die am besten geeignete Prothese auszuwählen. Nur so kann eine sichere Anwendung und ein langfristig optimales Ergebnis gewährleistet werden.
Abstract
Clinical/methodical issue
New technical developments in endovascular aortic repair (EVAR) have broadened the range of patients eligible for minimally invasive aneurysm treatment. Optimization of delivery sheaths and catheters by considerable downsizing of diameters, increase of pushability and stability combined with flexibility are important parameters.
Performance
Especially patients characterized by small and tortuous iliac access vessels can nowadays be treated by EVAR. Ease and effectiveness of applicability guarantee safety and quality improvement, which results in better treatment of patients. Progress in stent-graft design with integrated options for repositioning, active positioning and aneurysm sealing facilitate treatment of angulated vessel segments or hitherto unsuitable sealing zones.
Practical recommendations
Interventionalists have to be familiar with all available stent-graft materials and techniques. Profound knowledge helps to choose the best material for a patient’s individual anatomy, confident application and long-term satisfactory results.

Video 1 Die digitale Subtraktionsangiographie visualisiert die Injektionsmöglichkeit über den Einführkatheter (14 F) nach partieller Freisetzung der beiden korrekt zueinander ausgerichteten aortalen Komponenten des Altura™-Systemes (Iomeprol 270 mg Jod/ml; Volumen 10 ml, Injektionsgeschwindigkeit 20 ml/s). Anhand der röntgendichten Marker, die der Ummantelung unmittelbar proximal aufsitzen, ist eine akkurate infrarenale Positionierung auch bei ungünstiger proximaler Landezone möglich.
Video 2 Die Fluoroskopie demonstriert die Freisetzung einer iliakalen Komponente des Altura™-Systems. Die Implantation erfolgt von distal nach proximal. Wie beim aortalen Einführkatheter besteht die Möglichkeit zur Kontrastmittelinjektion. Damit ist ohne Schleusenwechsel eine zügige und genaue In-situ-Positionierung zum Erhalt der A. iliaca interna möglich, die hier im Video an der ostial gelegenen scholligen Kalzifikation zu erkennen ist. Die distale und proximale Stentstrebe der iliakalen Verlängerungen sind jeweils nicht ummantelt.
Video 3 Die Fluoroskopie zeigt den Magnetmechanismus der Anaconda™-Prothese, um einen innerhalb des kontralateralen Beinchens der Bifurkationsprothese bereits vorgelegten Cross-over-Draht zu koppeln. Über den eingefangen Draht ist in der Folge eine Katheterisierung des kontralateralen Beinchens möglich.
Video 4 Die digitale Subtraktionsangiographie demonstriert nach Implantation des Nellix®-Sytems die vollständige Ausschaltung des infrarenalen Aortenaneurysma unter Erhalt der Nierenarterien, der linken kaudalen schmalkalibrigen akzessorischen Nierenarterie und der Iliakalarterien.
Video 5 Korrespondierend zu Abb. 6 und Video 4 bestätigt die Computertomographie nach Implantation des Nellix®-Sytems die vollständige Ausschaltung des infrarenalen Aortenaneurysma unter Erhalt der Nierenarterien, der linken kaudalen schmalkalibrigen akzessorischen Nierenarterie und der Iliakalarterien. Nach mehreren Wochen reduziert sich der Kontrast des Polymers in den Kunststoffbeuteln durch Austritt des enthaltenen Jods.
Video 6 Die Zeitrafferaufnahmen zeigen die schrittweise Freisetzung der suprarenalen Fixierung und die Polymerfüllung der Abdichtungsringe der Ovation®-Prothese.
Video 7 Die Fluoroskopie zeigt die schrittweise Freisetzung der Zenith®-Prothese inklusive des kontralateralen Beinchens durch manuellen Rückzug der Einführschleuse unter Fixierung des Trägersystems.
