Zusammenfassung
Hintergrund
Die stetige Zunahme der Lebenserwartung unserer Bevölkerung bringt einen exponentiellen Anstieg proximaler Femurfrakturen mit sich. Gleichzeitig steigt die Zahl der Begleiterkrankungen dieser Patienten, so dass neue Anforderungen eine fortwährende Weiterentwicklung moderner Implantatsysteme zur osteosynthetischen Versorgung proximaler Femurfrakturen unabdingbar machen.
Material und Methoden
Wir untersuchten prospektiv 116 Patienten mit pertrochantären Femurfrakturen, welche mittels Targon PF über einen Zeitraum von 1 Jahr behandelt wurden. Die Indikation zur Verwendung des genannten Implantatsystems wurde bei instabilen Frakturtypen gestellt. Klinische Daten des operativen und postoperativen Verlaufs wurden prospektiv erhoben.
Ergebnisse
Das Durchschnittsalter der 116 überwiegend weiblichen Probanden war 77 ± 13,60 Jahre, der am häufigsten beobachtete Fraktursubtyp die 31-A1.2-Fraktur. Ein Drittel der Untersuchten war bereits präoperativ nur eingeschränkt gehfähig. Die Nachuntersuchungsrate betrug 55 %. Wir stellten postoperativ einen Abfall des modifizierten Harris-Hip-Scores um 22,7 % fest. Die 1-Jahres-Mortalität lag bei 21,6 %.
Schlussfolgerung
Unserer Ergebnisse zeigen eine geringe Rate perioperativer Komplikationen und Implantatversagen. Dennoch ist innerhalb eines Jahres ein typischer Rückgang der Patientenmobilität zu beobachten.
Abstract
Background
The steady rise in life expectancy of our population leads to an exponential increase in proximal femoral fractures. The growing increase of comorbidities in these patients requires continuous development of modern implant systems for internal fixation of proximal femoral fractures.
Materials and methods
In this study we enrolled 116 patients with pertrochanteric femoral fractures who were treated with a proximal femoral nail (Targon PF, Aesculap) over a period of 1 year. The indications for this implant system were set at unstable fracture types. Data of the operative and postoperative course were collected prospectively.
Results
The average age of the 116 predominantly female subjects was 77±14 years and the most commonly observed fracture subtype was 31-A1.2. The follow-up rate was 55 %. We observed a decrease in the postoperative modified Harris hip score of 22.7 %. The 1-year mortality was 21.6 %.
Conclusions
The results of this study showed a low rate of perioperative complications and implant loss anda decline in patient mobility was typically observed within 1 year.
