Zusammenfassung
Eine persistierende Dysphagie mit Stimmbandparese nach ventraler Instrumentierung einer Halswirbelsäulenfraktur bei vorbestehender diffuser idiopathischer Skeletthyperostose (DISH) ist eine seltene, aber für den jeweiligen Patienten dramatische Komplikation. Im Rahmen des geschilderten Fallbeispiels werden mögliche pathogenetische Faktoren diskutiert. Letztendlich sollte bei diesen Patienten eine sorgfältige und konsequente Abtragung der Spondylophyten erfolgen, um die laryngeale Dekompensation zu verhindern.
Abstract
Persistent dysphagia after ventral instrumentation of a patient with a cervical spine fracture and diffuse idiopathic skeletal hyperostosis (DISH, or Forestier’s disease) is a rare but dramatic complication. In this case report some pathogenetic factors are discussed. Accurate resection of the spondylophytes should be considered to avoid a ventral protrusion of the plate.
