Chronische Abdominalbeschwerden und Obstipation

Zusammenfassung

Chronische Obstipation kann ebenso wie das Auftreten einer Invagination ein wichtiger Hinweis auf eine strukturelle Darmpathologie sein. Es wird von einem 4 Jahre alten Mädchen berichtet, das zur Diagnostik bei chronischen Abdominalbeschwerden und Obstipation vorgestellt wurde. In der Sonographie fanden sich eine Raumforderung im rechten Oberbauch und eine ileokolische Invagination. Die Desinvagination erfolgte hydrostatisch; die anschließend durchgeführte Hydrosonographie erbrachte den Hinweis auf einen Kolonpolyp. Die Abtragung des singulären juvenilen Polypen konnte komplikationslos erfolgen. Postinterventionell war das Mädchen beschwerdefrei. Pathogenetisch ist anzunehmen, dass es durch den Polypen zu rezidivierenden kurzzeitigen ileokolischen Invaginationen kam, die für die Symptomatik ursächlich waren.

Abstract

Chronic constipation as well as the occurrence of intussusception can be clinical signs of an underlying intestinal pathology. The case of a 4-year-old girl is reported who presented with chronic abdominal pain and constipation. Ultrasonography showed a mass in the upper right abdomen and coexisting ileocolic intussusception. A hydrostatic reduction of the intussusception could be performed and hydrosonography subsequently revealed a polypoid structure located in the ascending colon. Colonoscopic ablation of the singular juvenile polyp was uneventful and after ablation the girl was free of complaints. Pathogenetically it could be assumed that the polyp led to recurrent short-term ileocolic intussusception which caused the abdominal pain and constipation.
