Zusammenfassung

Unter Kreislaufschwäche wird häufig eine symptomatische Hypotonie unterschiedlicher Ätiologie verstanden. Das klinische Resultat der Herz-Kreislauf-Dysregulation ist eine arterielle Hypotonie. Die Hypotoniemanifestation wird häufig in eine chronische und akute Verlaufsform unterteilt. Die chronische Hypotonie wiederum lässt sich ätiopathogenetisch in eine primäre und sekundäre Hypotonie untergliedern. Die akute Hypotonie wird meist mit einem Kreislaufschock gleichgesetzt. Während Patienten mit chronischer Hypotonie primär die Hausarztpraxis aufsuchen sollten, sollte eine Vorstellung von Patienten mit akut aufgetretener Hypotonie und oft schwerer klinischer Ausprägung über die Notaufnahme oder im Falle einer spezifischen Ursache direkt über die Akutklinik erfolgen. Eine Standardisierung von diagnostischen und therapeutischen Pfaden auf ambulanter und Klinikebene – nicht nur für das Management der Kreislaufschwäche – hätte neben einer Verbesserung der medizinischen Versorgung auch gesundheitsökonomische Vorteile.

Abstract

Circulatory insufficiency is often understood as symptomatic hypotension due to various causes. The clinical result of circulatory dysregulation is arterial hypotension. The manifestation of hypotension is often divided into chronic and acute forms. Chronic hypotension can be distinguished etiopathogenetically into primary and secondary hypotension. Acute hypotension is usually equated to circulatory shock. While patients with chronic hypotension should be seen primarily by their general practitioner, patients with acute hypotension and a frequently severe clinical manifestation should be evaluated in the emergency department or, if there is a specific cause, directly in the acute clinic. Standardization of diagnostic and therapeutic pathways in both out- and inpatient care—not only for the management of circulatory weakness—would be an improvement not only in regards to patient care, but also with respect to healthcare economics.
