Zusammenfassung
Hidradenitis suppurativa (HS) ist eine chronisch-entzündliche Haut- und Systemerkrankung, die mit erheblichen Beschwerden und einer deutlichen Einschränkung der Lebensqualität einhergeht. Trotz rezent gestiegenem Verständnis für die Erkrankung wird die Diagnose weiterhin über viele Jahre verzögert gestellt. Durch den verzögerten Zugang der Patient:innen zu einer geeigneten Therapie kommt es oft zur Progression der Erkrankung mit vermehrten operativen Eingriffen und dem Auftreten von möglichen Komorbiditäten. In den letzten Jahren hat sich das Verständnis der Pathophysiologie der HS verbessert, und in der Folge wurden auch moderne Therapeutika für die HS zugelassen. Die Therapie der HS ruht auf 3 Säulen: Operation, Antibiotika und Biologika. Zudem sollten Risikofaktoren wie Rauchen und Adipositas positiv beeinflusst werden. Für die individualisierte Betreuung der Patienten ist die Kenntnis der Komorbiditäten und ihrer interdisziplinären Therapie von Bedeutung.
Abstract
Hidradenitis suppurativa (HS) is a chronic inflammatory skin and systemic disease that is associated with considerable discomfort and a significant reduction in the quality of life. Despite a significantly increased understanding of the disease, the diagnosis is still delayed for many years. Delayed patient access to suitable treatment often leads to disease progression with increased surgical interventions and the occurrence of possible comorbidities. In recent years, there has been an improved understanding of the pathophysiology and, as a result the authorization of modern therapeutic agents for HS. The treatment of HS is based on three treatment pillars: surgery, antibiotics and biologics. Additionally, risk factors, such as smoking and obesity should be positively influenced. Knowledge of comorbidities and their interdisciplinary treatment is important for the individualized care of patients.

Welche Antwort zur Hidradenitis suppurativa ist richtig?
Hidradenitis suppurativa hat in Deutschland eine Prävalenz von ca. 5 %.
Männer sind öfter betroffen als Frauen.
Es wird von einer hohen Dunkelziffer ausgegangen.
Frauen haben ein schwereres Krankheitsbild als Männer.
Die Krankheit schreitet meist nicht weiter fort.
Ein 25 Jahre alter Patient stellt sich erstmals in Ihrer Sprechstunde vor und berichtet über rezidivierende schmerzhafte Knochen im Gesäßbereich. Sie klären den Patienten über seine Erkrankung auf. Welche Aussage trifft zu?
Nassrasuren helfen, die Ausbreitung der Erkrankung zu verhindern.
Erhöhtes Körpergewicht ist nicht mit einer Verschlechterung der Erkrankung assoziiert.
Mechanische Reize sowie enge Kleidung gelten nicht als Triggerfaktoren.
Es besteht eine Assoziation der Erkrankung mit aktivem Rauchen.
Im fortgeschrittenen (narbigem) Stadium ersetzen systemische Therapien mit Biologika zunehmend operative Therapien.

Die Diagnose erfolgt weiterhin ca. 10 Jahre verzögert.
Die Krankheit wird meistens gleich richtig diagnostiziert.
Durch die Verzögerung der Diagnose kommt es nicht zu Folgezuständen.
