Zusammenfassung
Notfälle nach indizierten und lege artis durchgeführten Injektionen und Infusionen sind in der Dermatologie selten. Die Embolia cutis medicamentosa ist eine seltene Komplikation nach intramuskulärer Injektion, die zu infarktähnlichen Hautnekrosen führt. Sie tritt nach intraglutäaler Injektion von Kortikosteroiden, Antiphlogistika, Antibiotika auf und wurde vereinzelt nach intraartikulärer Injektion sowie nach venöser Sklerosierungstherapie mit Polidocanol beschrieben. Das Hoigné-Syndrom gilt als pseudoanaphylaktische oder pseudoallergische Reaktion nach intramuskulärer Applikation von Procainpenizillin. Die neuropsychiatrische Reaktion setzt unmittelbar nach Injektion ein. Subakute Formen sind nach intramuskulärer oder oraler Gabe strukturell verwandter Antibiotika beschrieben. Der intraoperative Einsatz von Farbmarkierung mit Patentblau V ist mit einem ca. 1- bis 2%igen Risiko für allergische Reaktionen verbunden. Eine Überwachung des Patienten nach Injektion ist notwendig, um die innerhalb weniger Minuten einsetzende und potenziell lebensbedrohliche Soforttypreaktion rasch zu erkennen. Paravasation von Zytostatika ist eine gravierende iatrogene Komplikation in der Onkologie mit einer Häufigkeit von bis zu 6,5%. Kenntnisse über das Nekrosepotenzial der verwendeten Zytostatika, eine Notfallbox mit Anleitungen zu notwendigen Sofortmaßnahmen, substanzspezifischer Lokaltherapie/Antidote gehört zur Standardausstattung einer onkologischen Abteilung.
Abstract
Emergency situations following appropriately administered injections and infusions are an uncommon dermatologic problem. Embolia cutis medicamentosa is an uncommon complication following intramuscular injection which leads to infarct-like cutaneous necrosis. It may develop after gluteal injection of corticosteroids, anti-inflammatory agents and antibiotics and has rarely been described following venous sclerotherapy with polidocanol. Hoigné syndrome is a pseudoanaphylactic or pseudoallergic reaction following intramuscular injection of procaine penicillin, with neuropsychiatric problems developing immediately after the injection. Subacute forms following intramuscular or oral administration of structurally-related antibiotics. The intraoperative use of patent blue dye has a 1–2% risk of allergic reactions. The patient must be monitored carefully following injection for the onset the life-threatening immediate reaction. Extravasation of cytostatic agents is a critical iatrogenic problem in oncology with an incidence of up to 6.5%. Every oncologic department should have an understanding of necrotic risk of the various agents and an emergency kit with instructions for immediate steps, also including agent-specific antidotes as available.

Kindling (aus dem Englischen: to kindle = anfachen, zündeln) beschreibt ein Tiermodell der experimentellen Epilepsieforschung, bei dem durch repetitive unterschwellige Elektroimpulse nach einiger Zeit ein epileptischer Anfall durch niedrige Reize ausgelöst wird oder spontan auftreten kann. Kindling-Phänomene können auch durch chemische oder pharmakologische Reize induziert werden. Mit dem Modell wurden bestimmte Regionen des limbischen Systems für die Entstehung, Verstärkung und Ausbreitung überschießender Erregung identifiziert. Im Detail unterscheiden sie jedoch Tiermodell und epileptische Phänomene beim Menschen.
