Das Hoigné-Syndrom Kasuistik und aktueller Literaturüberblick

Zusammenfassung

Das heute als Hoigné-Syndrom bekannte Krankheitsbild einer akuten, pseudoanaphylaktischen bzw. pseudoallergischen Reaktion nach (intramuskulärer) Gabe von (prokainhaltigem) Penizillin G ist vorwiegend durch neuropsychiatrische Auffälligkeiten gekennzeichnet. Hierzu zählen unter anderem psychomotorische Erregungszustände, Ich-Störungen im Sinne eines Fragmentations- bzw. Depersonalisations- und Derealisationserlebens, Wahrnehmungsveränderungen in Form von Körperschemastörungen, akustische und optische Halluzinationen, multiple, oft panikähnliche Ängste bis hin zu massiver Todesangst sowie Bewusstseinstrübung und zerebrale Krampfanfälle. Neben der “klassischen”, perakuten Verlaufsform sind auch subakute und sog. “Latent-type-Verläufe” bekannt. Anhand einer typischen Kasuistik werden in einem aktuellen Literaturüberblick das klinische Erscheinungsbild, Hypothesen zur Ätiopathogenese und gegenwärtig verfügbare Behandlungsoptionen dieser nicht selten übersehenen Komplikation einer Penizillinantibiose näher dargestellt.

Summary

Hoigne's syndrome is currently considered a pseudoanaphylactic or pseudoallergic reaction following intramuscular and aqueous procaine penicillin administration. This disorder is characterized predominantly by neuropsychiatric alterations including severe psychomotor agitation with confusion, sensations of disintegration, depersonalization, and derealization, perceived changes of body shape, visual and auditory hallucinations, panic-like anxiety including fear of death as well as alterations of consciousness and seizures. Beside the “classic” immediate manifestation of Hoigne's syndrome, subacute forms as well as reactions of the so-called latent type are also known. Including a typical case report, we present a review of the currently available literature concerning clinical picture, hypotheses on origin, and possible therapy regimens of this underdiagnosed complication of antibiotic penicillin treatment.
