Abstract.
Introduction: Pulmonary nodular amyloidosis (PNA) is a phenomenon that is rarely diagnosed anywhere in the world. Methods: We report a case of a 63-year-old woman who smoked in whom a chest X-ray examination 5 years after diagnosis and radical treatment of a highly differentiated carcinoma of the corpus uteri showed multiple lung metastasis. To elucidate these findings by bronchoscopy and thoracoscopy we took a specimen from the right pleura and from one of the suspicious nodules, which were up to 3 cm in diameter; we also obtained some of the bronchial secretion. Results: The nodules were histopathologically diagnosed as PNA. Conclusion: Even if it is rare, PNA also belongs in the differential diagnosis of metastatic cancers. For us evidence of Pseudomonas fluorescens in the sputum is a reason for discussing a chronic, clinical unobtrusive local inflammation with hyperactivity of the B-cells as the hypothetical etiology of the amyloidomas.
Zusammenfassung.
Einleitung: Die pulmonal noduläre Amyloidose (PNA) ist ein weltweit äußerst selten diagnostiziertes Phänomen. Methoden: Wir berichten über den Fall einer 63 jährigen Raucherin, bei der eine Röntgenaufnahme des Thorax 5 Jahre nach Diagnose und radikaler Therapie eines hochdifferenzierten Carcinoms des corpus uteri das Bild einer multiplen pulmonalen Metastasierung bot. Zur Abklärung des Befunds entnahmen wir neben einer bronchoskopisch gewonnenen Sekretprobe auch thoracoskopisch Gewebe aus der rechten Pleura sowie aus einer der bis zu 3 cm im Durchmesser betragenden metastatisch anmutenden Veränderungen. Ergebnisse: Histopathologisch ergab sich das Vorliegen einer PNA. Schlussfolgerung: Wenn auch selten, so zählt auch die PNA zur Differentialdiagnose metastatischer Tumorerkrankungen. Den Nachweis von Pseudomonas fluorescens im Bronchialsekret nahmen wir zum Anlass, eine chronische, klinisch blande, lokale Entzündung mit B-Zell-Überaktivität als Ursache der Amyloidome hypothetisch zu diskutieren.
