Chirurgische Therapie der komplexen chronischen Visceralarterienischämie mit aortocoeliacalem, mesenterialem Patch-Bypass

Summary.

Based on two patients suffering from chronic visceral ischemia, the anatomic and pathophysiologic principles prior to surgery are discussed. Antegrade revascularization is supposed to be better hemodynamically. Complete revascularization of multiple arteries seems to be theoretically superior to one-vessel procedures. Within the bypass procedures the aortic-celiac-mesenteric patch bypass could be an excellent surgical possibility combining antegrade with simultaneous revascularization of celiac and superior mesenteric artery.

Zusammenfassung.

Am Beispiel von 2 Patientinnen mit klassischer Symptomatik einer chronischen komplexen Visceralarterienischämie werden anatomische sowie pathophysiologische Prinzipien als Voraussetzung für eine adäquate chirurgische Therapie erläutert. Die anterograde Revascularisation hat theoretisch aus hämodynamischer Sicht Vorteile. Eine Mehrgefäßrevascularisation erscheint dem Ein-Gefäß-Vorgehen hinsichtlich der Langzeitprognose überlegen. Innerhalb der Bypassverfahren könnte der aortocoeliacale, mesenteriale Patch-Bypass somit eine ideale Variante darstellen, da er eine anterograde mit einer simultanen Revascularisation von Truncus coeliacus und A. mesenterica superior vereint.
