Zusammenfassung
Zielsetzung der umfassenden monozentrischen Analyse waren aktuelle Aussagen zu Prognose des Mundhöhlenkarzinoms als eines der zehn häufigsten bösartigen Malignome des Menschen. In der Zeit von 1993 bis 1998 wurden alle aufgrund eines Plattenepithelkarzinoms der Mundhöhle behandelten Patienten hinsichtlich TNM-Stadium, Grading, operativer Strategie und Überlebenszeit analysiert und mittels einer Datenbank im Format SPSS 11.0 erfasst.
Bei 99 Patienten fand sich eine generelle 5-Jahres-Überlebenswahrscheinlichkeit von 57,3%. Das Überleben der Patienten war von der Tumorgröße und dem Ausmaß von Lymphknotenmetastasen abhängig: Bei den T1-Tumoren überlebten 80,1% (n=23) bei den T4-Malignomen hingegen nur 16,2% (n=28), beim N0-Stadium 68,7% (n=55) bei >N0 42,8%. Hoch differenzierte Karzinome hatten eine Überlebenswahrscheinlichkeit von 78,9% (n=26), die G2- und G3-Tumoren von nur 48,9%.
Tumorgröße und Lymphknotenmetastasen sind die entscheidenden klinischen Parameter für die Prognose des Mundhöhlenkarzinoms.
Abstract
The aim of the present study was to analyze different prognostic factors to calculate the overall survival in oral squamous cell carcinoma (OSCC). Samples retrospectively collected from 99 patients with primary OSCC were analyzed with regard to tumor node metastasis stage, grading, and 5-year survival time and summarized at an SPSS 11.0 databank. Treated were 72 men and 27 women (average age 59 years) due to oral squamous cell carcinoma. A general 5-year survival time of 57.3% was found. Patient survival depended on tumor size and the extent of lymph node metastasis: survival was 80.1% (n=23) for T1 tumors and only 16.2% (n=28) for T4 tumors, 68.7% (n=55) at the N0 stage and 42.8% at >N0 (n=44, χ2 test P=0.01, Fischer’s exact test P=0.014). Highly differentiated carcinomas (n=26) had a survival probability of 78.9% and G2 and G3 tumors of only 48.9% (n=73, χ2 test P<0.001). Tumor size and lymph node metastases are decisive.
