Chemotherapie und Radiochemotherapie bei Tumoren im Kopf-Hals-Bereich

Zusammenfassung

In der Behandlung von Kopf-Hals-Tumoren und insbesondere von Karzinomen der Mundhöhle ist die chirurgische Resektion die Therapie der ersten Wahl. Die zytostatische Chemotherapie allein ist für kurativ intendierte Behandlungen nicht geeignet, aber zur Palliation und zur zeitweiligen Begrenzung von Tumormanifestationen sinnvoll einsetzbar. Als adjuvante Maßnahme und in Verbindung mit der Radiotherapie kann die Chemotherapie jedoch die Überlebensrate und die Heilungsaussichten verbessern. In einer multizentrischen, randomisierten, prospektiven und kontrollierten DÖSAK-Studie wurden die Behandlungserfolgte nach alleiniger Operation und nach Operation in Kombination mit Chemotherapie und Radiotherapie verglichen. Die Life-table-Analyse verspricht statistisch gesicherte Überlebensvorteile für alle Tumorpatienten, die mit der Kombination aus Operation, Chemotherapie und Radiotherapie behandelt werden.

Summary

Standard curative therapy in head and neck cancer, especially in oral cavity carcinoma, consists, so far, of surgery. Until today, chemotherapy using standard drugs has failed to be effective beyond controlling cancer manifestations and palliation of symptoms. The introduction of chemotherapy and radiotherapy as adjuvant treatment, however, may improve overall survival and curative options. A multicentric randomized trial, initiated by DÖSAK, has compared results after radical surgery alone with results of combined preoperative radio-chemotherapy followed by radical surgery. The life table analysis substantiated improved survival rates in the group of patients whose treatment included chemotherapy and radiation.
