Zusammenfassung
In dieser Fallstudie wird über die OP-Disposition und -Planung in einem kommunalen Haus berichtet. Die Planung der OP-Kapazitäten erfolgt über ein Jahr im Voraus. Dabei geschieht das Controlling der OP-Auslastung nur mithilfe von 2 globalen Kennzahlen: a) die realisierte Abweichung von der erwarteten optimalen und im Voraus berechneten Auslastung, b) die Pünktlichkeit des Beginns der ersten Operation/Saal. Eine konsensorientierte Führung über einen OP-Koordinator, der durch umfassende Transparenz eine hohe Akzeptanz schafft, ist beim vorgestellten OP-Management-Ansatz zentral. Mithilfe dieser beiden Steuergrößen kann das operative Controlling transparent realisiert werden. Auf der Basis der vereinbarten Betriebszeiten wird die optimale Auslastung der zentralen OP-Einheit [OP_A(%)] mit einer einfachen Formel errechnet. So konnte die durchschnittliche Saalauslastung für alle Säle von 40% auf über 55% gesteigert werden. Für eine optimale Saalauslastung ist allerdings ein gelebtes und verbindliches OP-Statut neben einem von Einzelinteressen unabhängigen OP-Manager Voraussetzung. Mit diesem Konzept wurden im Kriterienkatalog für die Zertifizierung nach „Kooperation für Transparenz und Qualität im Krankenhaus“ (KTQ) mehr als 90% erreicht.
Abstract
We report the results of our study concerning the organisation of operating room (OR) capacity planned 1 year in advance. The use of OR is controlled using 2 global controlling numbers: a) the actual time difference between the expected optimal and previously calculated OR running time and b) the punctuality of starting thefirst operation in each OR. The focal point of the presented OR management concept is a consensus-oriented decision-making and steering process led by a coordinator who achieves a high degree of acceptance by means of comprehensive transparency. Based on the accepted running time, the optimal productivity of OR’s (OP_A(%) can be calculated. In this way an increase of the overall capacity (actual running time) of ORs was from 40% to over 55% was achieved. Nevertheless, enthusiasm and teamwork from all persons involved in the system are vital for success as well as a completely independent operating theatre manager. Using this concept over 90% of the requirements for the new certification catalogue for hospitals in Germany was achieved.
