Zusammenfassung
Fragestellung
In der vorliegenden Untersuchung wurde die Geschlechtsabhängigkeit von Propofolverbrauch und Aufwachzeiten nach Propofol-Remifentanil-Anästhesie bei standardisierter Anästhesietiefe untersucht.
Methodik
Nach Genehmigung durch die Ethikkommission wurden je 60 Männer und Frauen untersucht, die sich einem orthopädischen Eingriff unterziehen mussten. Bei jedem Patienten wurden EEG-Elektroden für Narcotrend- und BIS-Monitor nach Herstellerangaben platziert. Die Anästhesie wurde mit Remifentanil 0,4 µg/kg/min und einer Propofol-„target-controlled-infusion“ (TCI) mit einer Startkonzentration von 3,5 µg/ml eingeleitet. Intraoperativ erhielten alle Patienten 0,2 µg/kg/min Remifentanil. Die Propofolinfusion wurde in der Gruppe „Standardprotokoll“ nach den üblichen klinischen Kriterien (beide EEG-Monitore verdeckt) dosiert, in der Gruppe „BIS“ wurde ein BIS-Wert von 50 (Narcotrend verdeckt) und in Gruppe „Narcotrend“ wurde die Stufe D0 angesteuert (BIS verdeckt). Die Propofoldosierung wurde 15 min vor Operationsende reduziert, in der Gruppe „Standardprotokoll“ nach klinischen Kriterien, ansonsten entweder auf einen BIS-Zielwert von 60 oder auf die Narcotrend-Stufe C1. Die Zufuhr von Propofol/Remifentanil wurde zum Ende der Hautnaht gestoppt, die Aufwachzeiten gemessen und der Propofolgesamtverbrauch ermittelt.
Ergebnisse
Operationsdauer und Remifentanildosierungen waren in allen Gruppen vergleichbar. In der Gruppe „Standardprotokoll“ war der Propofolgesamtverbrauch bei männlichen und weiblichen Patienten nahezu identisch; die Aufwachzeiten bei den männlichen Patienten waren aber signifikant länger. In den beiden EEG-gesteuerten Gruppen war der Propofolverbrauch bei den männlichen Patienten geringer bei gleichzeitig geringfügig längeren Aufwachzeiten. Bei den weiblichen Patientinnen musste eine höhere Propofol-TCI-Konzentration angesteuert werden, um die gleichen BIS-Werte oder Narcotrend-Stufen zu erreichen.
Schlussfolgerung
Aufwachzeiten und Propofolverbrauch zeigen geschlechtsabhängige Unterschiede, sowohl bei klinischer als auch bei EEG-gesteuerter Anästhesieführung.
Abstract
Introduction
We investigated gender differences of drug consumption and recovery times for propofol-remifentanil anaesthesia.
Methods
Adult patients scheduled for minor orthopaedic surgery were randomised to receive a propofol-remifentanil anaesthesia controlled either by EEG monitoring (Narcotrend or BIS) or solely by clinical parameters. Anaesthesia was induced with remifentanil 0.4 µg/kg/min and a propofol target-controlled infusion (TCI) at 3.5 µg/ml. After intubation remifentanil was reduced to 0.2 µg/kg/min whereas propofol TCI was adjusted according to clinical parameters or to the following EEG target values: during maintenance to “D0” (Narcotrend) or “50” (BIS), 15 min before the end of surgery to “C1” (Narcotrend) or “60” (BIS). Recovery times were recorded and average normalised propofol consumption was calculated from induction and maintenance doses.
Results
A total of 60 male and 60 female patients completed the study. Gender differences were observed for recovery times (with standard practice) and for propofol consumption (with BIS monitoring). In the standard protocol group, propofol consumption was nearly identical for male and female patients whereas recovery times were significantly longer in the male group. In both EEG-guided groups propofol consumption was less for male patients while recovery times were slightly longer. In the group of female patients higher propofol TCI concentrations had to be used to reach the same BIS or Narcotrend values.
Conclusion
With propofol-remifentanil anaesthesia, gender has impact on recovery times and propofol consumption. If the same amounts of propofol are applied, males awake later, with BIS or Narcotrend monitoring males receive less propofol for comparable EEG effects.
