Kasuistik
Ein 68-jähriger Patient stellt sich zur internistisch-kardiologischen Abklärung vor. Er berichtet über reduzierte körperliche Leistungsfähigkeit sowie Schwindelattacken, die ohne besondere Vorankündigung auftreten und etwa 15–20 s anhalten würden. Vor 10 Jahren erlitt er einen transmuralen Hinterwandinfarkt. Bei der damals diagnostizierten koronaren Dreigefäßerkrankung wurde eine operative Revaskularisierung mittels koronarvenöser Bypasschirurgie durchgeführt. Aufgrund erneuter Angina-pectoris-Symptomatik erfolgte vor 1 Jahr eine erneute Koronarangiographie, bei der sich ein Verschluss des Venenbypasses zur rechten Koronararterie sowie drei offene Bypässe darstellten.
Der körperliche Untersuchungsbefund zeigt einen 180 cm großen und mit 95 kg übergewichtigen Patienten. Die Herzschlagfolge ist regelmäßig. In Linksseitenlage ist ein bandförmiges Systolikum (3/6) mit Fortleitung in die linke Axilla auskultierbar. In der Fahrradergometrie kann sich der Patient bis maximal 75 W belasten. Der Abbruch erfolgt bei zunehmender Dyspnoe. Während der Belastung treten gehäuft monomorphe ventrikuläre Extrasystolen auf, welche jedoch in der Erholungsphase wieder verschwinden. In der transthorakalen Echokardiographie findet sich eine stark reduzierte linksventrikuläre Funktion (LVEF 35%) mit inferiorer Akinesie. Zudem besteht eine Mitralklappeninsuffizienz Grad II. Das Zwölf-Kanal-EKG weist einen kompletten Linksschenkelblock sowie einen AV-Block I. Grades (PQ-Zeit 210 ms) bei Sinusrhythmus auf. Im daraufhin durchgeführten 24-h-Langzeit-EKG zeigen sich gehäuft kurze ventrikuläre Salven, jedoch keine anhaltenden ventrikulären oder supraventrikulären Tachykardien, Bradykardien oder Pausen > 2,5 s. In der elektrophysiologischen Untersuchung (EPU) finden sich eine erhöhte ventrikuläre Vulnerabilität und Induzierbarkeit von anhaltenden Kammertachykardien. Es erfolgt die Implantation eines biventrikulären Kardioverter-Defibrillator-(ICD-)Aggregats.
