Zusammenfassung
Analfisteln sind ein häufig auftretendes Krankheitsbild. Die meisten Fisteln sind kryptoglandulären Ursprungs und haben unterschiedliche Ausbreitungswege in Beziehung zum Schließmuskel. Die Einteilung von Analfisteln erfolgt in der Regel gemäß der modifizierten Klassifikation nach Parks. Für die Behandlung stehen mehrere Operationsverfahren zur Verfügung. Der Advancement-Flap ist neben der Fistelexzision mit Sphinkterrekonstruktion und dem LIFT-Verfahren (Ligatur des intersphinktären Fisteltrakts) eine Methode zur Sanierung transsphinktärer und suprasphinktäre Analfisteln. Die Erfolgsrate beträgt 60–80 %. Die Technik des Advancement-Flap wird in verschiedenen Modifikationen angewendet. Die vorliegende Arbeit stellt anhand eines Fallbeispiels die einzelnen Operationsschritte dar und gibt Hinweise für verschiedene Verfahrensweisen.
Abstract
Anal fistulas are a frequently occurring clinical condition. Most of the fistulas are of cryptoglandular origin and have different tracts in relation to the sphincter muscle. Fistulas are usually classified using the modified classification according to Parks. Several surgical procedures are available. In addition to fistula excision with sphincter reconstruction and the LIFT procedure (ligation of intersphincteric fistula tract), the advancement flap is a method for the reconstruction of transsphincteric and suprasphincteric anal fistulas. The success rate ranges from 60 to 80%. The advancement flap technique is used in various modifications. This article describes the surgical procedure step-by-step and presents different approaches using a case study as an example.
