Zusammenfassung
In den Jahren 1986 bis 1988 wurden 3 Patienten mit Aderhautmelanomen einer Protonenbestrahlung zugeführt. Ein Melanom war mittelgroß (Prominenz: 4 mm), zwei groß (Prominenz: 6,5 und 8,7 mm). Zwei der 3 Augen waren funktionell einzige Augen. Von einer Brachytherapie mit episkleralen Beta-Plaques wurde Abstand genommen, da alle Tumore am hinteren Pol in unmittelbarer Nähe von Makula oder Papille lokalisiert waren. Die Melanome wurden auswärts nach vorhergehender Lokalisation mit Tantalringen mit Protonen bestrahlt, wobei in 4 aufeinanderfolgenden Tagen insgesamt je 60 CGE (Cobalt Gray Äquivalent) eingestrahlt wurden. Im Nachbeobachtungszeitraum (mindestens 4 Jahre, Mittel: 4,8 Jahre) entwickelten beide Augen mit den primär großen Melanomen schwerste radiogene Komplikationen, das Auge mit dem größten Tumor mußte nach 48 Monaten wegen einer Phthisis bulbi enukliert werden. Metastasen konnten im Beobachtungszeitraum jedoch nicht beobachtet werden.
Die Indikation zur Bestrahlung von Melanomen am hinteren Pol mit Protonen sollte auf jene Patienten beschränkt bleiben, die eine Enukleation ablehnen, durch episklerale Plaques nicht adäquat versorgt werden können und über die Möglichkeit des Verlustes des Sehvermögens informiert wurden.
Summary
Three patients with uveal melanomas were treated abroad with proton beam irradiation. One tumor was medium-sized (prominence: 4 mm), two were large-sized (prominence: 6.5 and 8.7 mm). Two of the 3 eyes were functionally single eyes. All tumors were located at the posterior pole in vicinity to the macula or the optic disc. Fearing to damage these structures with a beta-plaque, proton beam irradiation was elected for treatment. After suturing of 4 tantalum rings 60 CGE (cobalt gray equivalent) were delivered in 4 equal treatments during 4 consecutive days. The minimum of follow-up for each patient is 4 years (average: 4.8 years). Functionally we lost both eyes with the large-sized tumors due to severe radiogenic side-effects, the eye containing the largest tumor requiring enucleation after 48 months because of phthisis bulbi. No metastases developed up to this time.
Proton beam irradiation for melanomas at the posterior pole should be restricted to patients, who refuse enucleation, cannot be treated sufficiently with an episcleral plaque and accept the likelihood of loss of central vision.
