Zusammenfassung
Im Hinblick auf die heute immer mehr aktualisierte Problematik der Frühtrabekulektomie als Initialtherapie frisch aufgedeckter primärer Offenwinkelglaukome wurde das Krankengut von 300 Trabekulektomien der Jahre 1985 bis 1988 bei einer durchschnittlichen Nachbeobachtungszeit von 2,5 Jahren retrospektiv analysiert. Das Nachlassen des drucknormalisierenden Effekts von 84% im ersten postoperativen Jahr auf 69%, durchschnittlich 2,5 Jahre postoperativ, die Erfordernis einer medikamentösen Zusatztherapie von 10% im ersten postoperativen Jahr gegenüber 25% durchschnittlich 2,5 Jahre postoperativ und das damit verbundene Risiko eines weiteren Gesichtsfeldverfalles sowie die beträchtliche Zunahme präoperativ bestehender Linsentrübungen in 13% schränken die Indikation der Frühtrabekulektomie auf bestimmte Patientengruppen ein: auf alle Patienten mit einer Lebenserwartung von mindestens 5 Jahren, Patienten mit Medikamentenunverträglichkeit oder non-compliance, auf Augen mit einem Intraokulardruck von 30 mmHg und mehr trotz medikamentöser Therapie bei normalem Gesichtsfeld, auf Augen mit Intraokulardruckwerten von 26–30 mmHg bei Skotomen, die bis auf 10° an den Fixationspunkt heranreichen und auf Augen mit Intraokulardruckwerten von 21 – 25 mmHg bei Skotomen, die 5 – 9° an den Fixationspunkt heranreichen.
Bei allen übrigen Ausgangssituationen empfiehlt sich weiterhin das konventionelle Vorgehen, nämlich die zeitlich unbegrenzte Ausschöpfung medikamentöser Behandlungsmodalitäten und der Lasertrabekuloplastik bzw. erst als Ultima Ratio die Trabekulektomie.
Summary
We have investigated 300 patients who underwent trabeculectomy between 1985 and 1988 to study the increasingly topical problem of early trabeculectomy as an initial therapy immediately after detection of primary open-angle glaucoma. This patient collective was retrospectively analyzed over an average follow-up observation period of 2.5 years.
The indication of early trabeculectomy should be limited to specific patient groups due to a decrease of the pressure-normalizing effect from 84% in the first postoperative year to 69% after an average of 2.5 years postoperatively, the elevated requirement of an additional medical therapy in 10% of patients in the first year postoperatively to an average of 25% 2.5 years after trabeculectomy and the associated risk of developing scotomas as well as the considerably increased incidence of postoperative cataracts in 13% of patients. These specific patient groups comprise all patients with a life-expectancy of at least 5 years, patients with drug incompatibility or non-compliance, patients with an intraocular pressure of 30 mmHg and more despite medical therapy with normal visual fields, patients with intraocular pressures between 26 and 30 mmHg and scotomas reaching 10° near the fixation point, and patients with intraocular pressures of 21–25 mmHg and scotomas reaching 5–9° near the fixation point.
With all other diagnostic settings we recommend to employ the conventionally established procedures, i.e. to apply medical treatment modalities without any time limit and to use laser trabeculoplasty. In these cases trabeculectomy as such should only be used as the ultimate choice.
