Operatives Vorgehen in der onkologischen Kolonchirurgie bei hoher Komorbidität

Zusammenfassung

Eine zunehmende Anzahl an Patienten mit einem Kolonkarzinom müssen im Zustand der Gebrechlichkeit für eine Operation evaluiert werden. Dabei korreliert die Gebrechlichkeit stark mit einer erhöhten therapieassoziierten Komplikationswahrscheinlichkeit. Hieraus resultiert eine Verschlechterung der Therapie-Risiko-Nutzenbilanz für diese Patientengruppe und erhöht den Anspruch an die Indikationsstellung insbesondere, wenn die Patientenautonomie adäquat berücksichtigt werden soll. Präkonditionierung des Patienten, die Verfahrenswahl mit Resektionsausmaß und die Maßnahmen zur Rehabilitation müssen alle so gewählt werden, dass ein möglichst hoher Wert der Therapie resultiert. Dabei ist zu berücksichtigen, dass auch ältere Patienten gegenüber den Nicht-Operierten von einer chirurgischen Therapie mit kurativer Zielsetzung einen erheblichen stadienabhängigen Überlebensvorteil haben.
