Zusammenfassung
Hintergrund:
LASIK-Operationen haben in den letzten Jahren stark zugenommen, mit der Anzahl der Eingriffe und des Beobachtungszeitraumes steigen aber auch die Komplikationen.
Fallbericht:
Wir berichten über eine 58-jährige Patientin mit einer ursprünglich hohen Myopie, die nach einer beidseitigen emmetropisierenden Kataraktoperation am amblyopen Auge zusätzlich eine LASIK-Behandlung erhielt. Nach dem Eingriff entwickelte sie nicht nur eine Keratektasie, sondern auch eine konservativ nicht beherrschbare Augendruckerhöhung, so dass eine fistulierende Operation durchgeführt werden musste. Trotz befriedigender Druckeinstellung ist die Patientin an diesem Auge in der Zwischenzeit erblindet.
Diskussion:
Durch die hohen Druckschwankungen im Bulbus während des Ansetzens der Mikrokeratomplatte könnten Schäden im Trabekelwerk entstehen, die im weitesten Sinne mit einem Kontusionskammerwinkel vergleichbar sind. Unsere Patientin zeigte am betroffenen Auge solche Veränderungen.
Schlussfolgerung:
Das Zusammentreffen mehrerer prädisponierender Faktoren (falsch niedrig gemessene Augendruckwerte bei verdünnter Hornhaut aufgrund der LASIK-Behandlung und der Keratektasie, Prädisposition für ein Glaukom und die Druckbelastung durch das Mikrokeratom) hat zu dem tragischen Ausgang der Behandlungen geführt. Eine LASIK-Behandlung an einem amblyopen und vorgeschädigten Auge ist sicher ein unnotwendiger Eingriff und stellt unserer Meinung nach eine Kontraindikation dar.
Summary
Purpose:
LASIK surgery increased enormously in the last years. The number of operations and the longer observation period lead to an amount in complication rate.
Case report:
A 58 years old woman, originally high myopic, got cataract surgery and afterwards a LASIK treatment on the amblyopic eye. She did not only develop keratectasia but also a not controllable high intraocular pressure, only treatable by an IOP lowering operation. Although the intraocular pressure is in normal range now, the patient got blind on this eye in the meantime.
Discussion:
While fixing on and removing the microceratome plate from the cornea by vacuum, the globe is exposed to high fluctuations of intraocular pressure. This could cause damage in the trabecular meshwork as we know it from the angle recession glaucoma. Some of these alterations were found in the gonioscopy of our patient.
Conclusions:
The addition of several predisposing facts (falsly measured normal intraocular tension of the thinned cornea because of the LASIK treatment and the keratectasia, disposition for glaucoma and the pressure waves during the fixation of the microkeratome) led to the poor result of all the treatments. In our opinion, LASIK surgery should not be performed in an amblyopic and disadvantaged eye to avoid such a tragic outcome.
