Keratoplastik á chaud bei therapierefraktärer Keratitis mit drohender oder erfolgter Hornhautperforation

Zusammenfassung

Bei 11 Patienten mit drohender oder erfolgter Hornhautperforation als Folge von bakteriellen Hornhautgeschwüren (6 Fälle), metaherpetischen Ulcera (3 Fälle), einer Pilzinfektion und einer Randfurchenkeratitis bei rheumatoider Arthritis (je 1 Fall) wurde zur Erhaltung des Auges eine Keratoplastik á chaud durchgeführt. Bei 6 Patienten (55%) blieb des Transplantat innerhalb einer Nachbeobachtungszeit von 12–30 Monaten klar. 2 dieser Patienten erreichten eine Sehschärfe von 0, 5, 4 weitere einen Visus von 0, 1 oder besser. Drei Patienten waren zur Nachkontrolle nicht mehr erschienen. Bei 1 Patienten mußte wegen eines Rezidives einer Staphylococcus aureus Infektion bei Dakryostenose 3 Monate nach erfolgreicher Keratoplastik á chaud das Auge enucleiert werden.

Summary

Tectonic penetrating keratoplasty was performed in 11 patients with corneal perforation or impending corneal perforation due to bacterial corneal ulcers (6 cases), metaherpetic corneal ulcers (3 cases), mycotic keratitis and keratitis associated with rheumatoid arthritis (one case respectively).

In 6 patients (55%) the corneal grafts remained clear for a period of follow-up ranging from 12 to 30 months. 2 of these patients achieved a visual acuity of 0.5 and 4 patients a visual acuity of 0.1 or better. 3 patients missed follow-up visits.

One patient underwent enucleation 3 months after successful tectonic keratoplasty due to recurrence of Staphylococcus aureus keratitis associated with stenosis of the lacrimal drainage system.
