Zusammenfassung
Ziel der Studie war es, das Zentrierungsverhalten und die Nachstarinzidenz von zwei Silikonfaltlinsenmodellen mit unterschiedlich rigiden Haptiken zu vergleichen. Dazu wurden 81 Linsen vom Typ SI30 mit Polypropylenhaptiken und 35 Linsen vom Typ SI40 mit PMMA-Haptiken, die in ”no-stich-Technik“ in den Kapselsack eingesetzt worden waren, retrospektiv nachkontrolliert. Der durchschnittliche Nachbeobachtungszeitraum betrug bei den SI30-Linsen 22 ± 12 (SD) Monate, bei den SI40-Linsen 4 ± 2 (SD) Monate.
Nach Ausschluß der Patienten mit Netz- und Hornhauterkrankungen sahen 87% der Patienten mit SI30- und 85% der Patienten mit SI40-Linsen mit einer Sehschärfe von mindestens 0,8. Die Dezentrierung betrug bei der SI30 durchschnittlich 0,47 ± 0,30 (SD) mm, bei der SI40 0,45 ± 0,30 (SD) mm (p = 0,88). Eine YAG-Laser-Nachstardiszision war in 3,7% bei der SI30 und in 2,9% bei der SI40 erforderlich. Bei weiteren 6,2% der Patienten mit SI30- und 8,6% mit SI40-Linsen kam es zum Auftreten eines leichten fibrotischen Nachstars im Zentrum der Hinterkapsel, der jedoch zu keiner wesentlichen Visusbeeinträchtigung führte (Visus ≥ 0,6).
Trotz der unterschiedlichen Rigidität der Haptiken war weder bei der Nachstarinzidenz noch beim Zentrierungsverhalten ein signifikanter Unterschied zwischen beiden Modellen festzustellen. Verglichen mit früheren Studien über faltbare Hinterkammerlinsen war bei beiden Modellen die YAG-Laser-Nachstarrate erfreulich niedrig. Extreme Dezentrierungen von mehr als 1,5 mm, wie sie zum Beispiel bei Modellen im Kahndesign auftreten können, wurden nicht beobachtet.
Summary
The intention of this study was to compare the centration and the incidence of secondary cataracts of two types of silicone lenses, which differ in the rigidity of their haptics.
We evaluated retrospectively 81 SI30-lenses with Polypropylenhaptics and 35 SI40-lenses with PMMA-haptics, which had been implanted during small incision cataract surgery into the capsular bag. The mean follow-up was 22 ± 12 (SD) months with the SI30- and 4 ± 2 (SD) months with the SI40-lenses. 87% of the patients with SI30- and 85% with SI40-lenses had a visual acuity of at least 0.8 (patients with retinal and corneal diseases were excluded from this statistics). Mean decentration was 0.47 ± 0.3 (SD) mm with the SI30-, and 0.45 ± 0.3 (SD) with the SI40-lenses (p = 0.88). 3.7% of the patients with SI30- and 2.9% of the patients with SI40-lenses needed a YAG-laser-capsulotomy. A mild fibrotic opacification in the center of the posterior capsule, which had almost no influence on the visual acuity (v ≥ 0.6), was found with 6.2% of the SI30- and 8.6% of the SI40-patients.
Despite the different rigidity of the haptics of the two lensmodells, no significant difference concerning decentration and posterior capsule opacification could be found. Compared with earlier studies on flexible lenses, the incidence of PCO requiring a YAG-laser-capsulotomy was with both types gratifyingly low. Extreme decentrations of more than 1.5 mm, was they may occur e.g. with lenses in flange design, did not occur.
