Zusammenfassung
Obwohl durch die gezielte Laserkoagulation von subretinalen Neovaskularisationen bei altersbedingter Makuladegeneration (AMD) ein rasches Absinken des zentralen Sehvermögens in der Mehrzahl der Fälle vermieden werden kann, wird die Entwicklung rezidivierender Gefäßneubildungen im postoperativen Verlauf nicht verhindert. In einem Krankengut von 50 Augen von 48 Patienten, die zwischen April und Oktober 1988 erstmals einer Laserbehandlung zugeführt wurden, traten in 27 Augen Rezidive auf, die je nach ihrer Lokalisation in 7 Gruppen eingeteilt wurden: subfoveal-, am Rand-, zentral/peripher im selben/ in anderen Quadranten zur letzten Koagulation gelegene und multipel aufgetretene Neovaskularisationen.
Die häufigsten Rezidive (21,3%) wurden am Rand einer koagulierten Neovaskularisation und peripher in anderen Quadranten (18,7%) nachgewiesen, selten traten postoperative Neovaskularisationen subfoveal auf (1,3%). Rezidive in den Gruppen peripher im selben Quadranten (16%), multipel (16%), zentral im selben (14,7%) bzw. in anderen Quadranten (12%) wurden in annähernd gleicher Zahl gefunden.
Bezüglich des Zeitpunktes bildeten sich Rezidive peripher im selben Quadranten (Ø 2,5 m) und am Rand (Ø 2,7 m) bald nach der letzten Laserkoagulation aus, im weiteren Verlauf Stellten wir vermehrt multipel auftretende Rezidive (Ø,7 m) und Rezidive zentral im selben Quadranten (Ø 4 m) fest. Im Durchschnitt später wurden Rezidive peripher (Ø 5,7 m) bzw. zentral (Ø 6,5 m) in anderen Quadranten nachgewiesen.
Unsere Analyse sowie der Vergleich von Ergebnissen anderer Autoren zeigen, daß der Verlauf einer exsudativen Makuladegeneration keiner vorgegebenen Gesetzmäßigkeit unterliegt und keine prognostische Aussage zuläßt.
Summary
Although effective laser treatment of subretinal neovascular membranes in AMD reduces the risk of severe central visual loss the development of recurrent neovascularization cannot be avoided in the postoperative period.
In a series of 50 eyes of 48 patients who underwent laser coagulation for the first time between April and October 1988 we found recurrent neovascularizations in 27 eyes (23 patients) which we divided into 7 groups depending on their location: subfoveal, marginal, central/peripher in the same/ in other quadrants to the last laser scar und multiple neovascular mambranes. Most recurrent neovascularizations (21.3%) were proven at the margin of a coagulation scar and peripher in other quadrants (18.7%), rarely they appeared subfoveally (1.3%). Recurrent neovascularizations peripher in the same quadrant (16%), multiple (16%), central in the same quadrant (14.7%) and in other quadrants respectively (12%) were found in nearly the same number. Relating to the time of their appearance recurrent neovascular membranes developed early after the last laser treatment peripherally in the same quadrant (Ø 2.5 m) and marginally to the sacr (Ø 2.7 m); in the later course we found multiple recurrent neovascularizations (Ø 3.7 m) and those centrally in the same quadrant (Ø 4 m). On a late average the new membranes were proven in other quadrants peripher (Ø 5.7 m) and central (Ø 6.5 m) to the last one. Our analysis and the comparison of the resutls of other authors show that the course of exudativ macular degeneration does not follow a pattern so that its prognosis can hardly be predicted.
