Zusammenfassung
Die Implantation von nahtfixierten Hinterkammerlinsen (HKLs) wird zunehmend propagiert. Bevorzugt wird dabei die transsklerale Fixation im Sulcus ciliaris. Inwieweit solcherart fixierte HKLs in den Fällen, wo die übliche Fixation einer HKL nicht mehr möglich ist, die modernen kammerwinkelgestützten Vorderkammerlinsen verdrängen wird, ist noch ungewiß, auch wenn die bisher gesammelten klinischen Erfahrungen dafür sprechen.
Gegenstand dieser Arbeit sind Fälle, wo diese Methode die einzige Alternative der Aphakiekorrektur darstellte. Die unterschiedlichen und meist komplexen Ausgangssituationen zwangen zum Einsatz situationsangepaßter technischer Varianten. In den Fällen, die eine gleichzeitige Keratoplastik erforderten, kam eine temporäre Silikonkeratoprothese zum Einsatz. Operationsmethode, Verlauf und erzielte Resultate werden geschildert. Das morphologische Ergebnis war mit einer Ausnahme zufriedenstellend. Der Fall einer wiederaufflammenden Uveitis zeigt jedoch, daß bei entsprechender Disposition mit einer Schrankenstörung zu rechnen ist, weswegen in solchen Fällen von der Sulkusfixation einer HKL Abstand genommen werden sollte. Die Sehschärfe konnte in 10 der 14 Fälle verbessert werden, in den übrigen vier Fällen ist das geringe Sehvermögen durch die vorbestehende Pathologie erklärt.
Summary
In the absence of capsular and zonular support implantation of suture-fixated posterior chamber lenses is increasingly popularized. The preferred method is transscleral fixation in the ciliary sulcus. Though clinical results do indicate this it still remains to be established in how far this method will replace modern angle-fixated anterior chamber lenses. This paper deals with those cases where a suture-fixated PCL was the only alternative for aphakic correction. The differing and often complex pathologies necessitated an individual technical approach. In cases those requiring simultaneous keratoplasty a temporary silicone keratoprosthesis was used. This paper reports on the surgical technique, the operative and postoperative course and the results obtained. Morphological results were satisfactory in most cases. However, the case of a reactivated uveitis demonstrates that the blood-aqueous barrier may be considerably and permanently compromized in predisposed eyes. Therefore, every effort should be made to detect and exclude these cases. Visual acuity improved in 10 out of the 14 cases.
In four eyes poor vision was explained by the preexisting pathology.
