Ontwikkeling en behandeling van encopresis bij adolescenten

Abstract

Encopresis is een typisch gedragsprobleem van kinderen, dat slechts sporadisch bij adolescenten gesignaleerd wordt. Aan de hand van literatuur en casuïstiek worden de typische kind- en gezinskenmerken van encopretici beschreven. Er blijkt een opvallend consistent beeld te bestaan van passiviteit en grote ‘geslotenheid’, niet alleen van de adolescent, maar ook van zijn gezin. Aan de hand van twee gevalsbeschrijvingen wordt een en ander geïllustreerd. Therapeutisch impliceert de behandeling van encopresis bij adolescenten een directieve werkwijze die tot doel heeft de zelfwerkzaamheid te bevorderen. Met behulp van training in sociale vaardigheden moet de adolescent voorts een sociaal netwerk kunnen opbouwen. In de discussie wordt de hypothese van een gegeneraliseerde psychosociale ontwikkelingsachterstand bij deze adolescenten toegelicht en uitgewerkt.

  1. De leeftijdsgrens wordt in de DSM-IV classificatie gesteld op ten minste vier jaar met een frequentie van minimaal één keer per maand gedurende minstens drie maanden. Er wordt geen onderscheid gemaakt in het daadwerkelijk zich bevuilen met ontlasting en sporen van verloren ontlasting in de broek.

  2. De patiënt is nooit zindelijk geweest (continu incontinen = rimaire vorm) en de faecale incontentinentie gaat (blijkens lichamelijk onderzoek of anamnese) gepaard met obstipatie en overloopincontinentie (zie Bosch, 1991, DSM-IV, 1994). Met overloopincontinentie wordt bedoeld ‘een constant onwillekeurig doorsijpelen van faeces, gerelateerd aan de verstopping’ (Bosch, 1984).
