Summary
Approximately 1600 children with a congenital heart defect are born in the Netherlands every year. The presentation of these children will be diverse and sometimes misleading, because of the many different heart defects possible. Moreover, not every child with a congenital heart defect will present in the first days of life. The first interventions have to be adequate and determine largely the clinical course. We present an overview of the most frequently seen congenital heart defects and their first presentation. We discuss initial treatment, with special attention to the indications for oxygen supply, prostaglandin therapy and criteria for safe transport.
Samenvatting
In Nederland worden per jaar ongeveer 1600 kinderen geboren met een aangeboren hartafwijking. Omdat het aantal verschillende diagnosen groot is, zal de presentatie van deze kinderen divers zijn en soms ook misleidend. Bovendien zal niet elk kind met een aangeboren hartafwijking zich al vroeg na de geboorte presenteren. De eerste opvang dient adequaat te zijn en bepaalt in belangrijke mate het klinische beloop. Er wordt een overzicht gegeven van de meest voorkomende hartafwijkingen en de klinische presentatie, en er worden adviezen gegeven inzake de eerste opvang. Aandacht wordt besteed aan de indicaties voor zuurstof- en prostaglandinetoediening. Daarnaast wordt ingegaan op de richtlijnen voor een veilig transport.
