Les gastropathies hyperplasiques se manifestent par des modifications du relief muqueux se traduisant par des plis monstrueux, cérébroïdes ou par la conjonction de plis hypertrophiés, de nodules et d’érosions.
Elles peuvent correspondre soit à des affections néoplasiques telles que la linite plastique, le lymphome..., soit à des affections inflammatoires ou dégénératives. Le présent travail porte sur le domaine des gastropathies hyperplasiques non tumorales. Les auteurs, se basant sur leur expérience de la gastrite lymphocytaire et sur les études qu’ils ont entreprises sur la maladie de Ménétrier proposent une nouvelle classification des gastrites hyperplasiques précisant mieux les entités histopathologiques qui forment le substrat de ces affections. Les gastropathies hyperplasiques habituellement rencontrées sont la gastrite varioliforme, la maladie de Ménétrier et la maladie de Crohn.
L’aspect histologique de la gastrite varioliforme diffuse ou fundique correspond à l’entité décrite sous le terme de gastrite lymphocytaire. Elle se diftérencie nettement de la gastrite varioliforme antrale dont la traduction histologique est hétérogéne, comprenant un groupe d’affections variées telles que la gastrite à hélicobacters, la gastrite chimique (reflux biliaire)...
La maladie de Ménétrier est une maladie rare caractérisée par un ensemble de symptômes cliniques, endoscopiques et radiologiques ; certains critères histologiques, en particulier, l’infiltrat lymphocytaire intraépithélial suggèrent une relation entre cette maladie et la gastrite lymphocytaire.
Quant à la maladie de Crohn, dans sa localisation gastrique, les lésions endoscopiques sont rarement fundiques ou diffuses et s’identifient à la gastrite varioliforme antrale. Le diagnostic repose sur la notion de lesions iléocoliques concommitantes ; d’un point de vue histologique, le seul élément discriminant est le granulome à cellules géantes été pithéloïdes à l’exclusion du microgranulome.
Summary
The term hypertrophic gastropathy has been used for many years to describe more generally any condition accompanied by giant folds nodules or erosions. Hypertrophic gastropathies can correspond either to neoplastic conditions such as linitis plastica or lymphoma or to inflammatory or degenerative diseases. The present article concerns non tumoral hypertrophic gastropathies. The description of lymphocytic gastritis added to studies of Menetrier’s disease have allowed a better specification of various entities leading to a new and more coherent classification. The hypertrophic gastropathies usually encountered have varioliform gastritis, Menetrier’s disease and Crohn’s disease.
Diffuse or fundic varioliform gastritis corresponds to the histological entity described by the term lymphocytic gastritis. It is clearly differentiated from antral varioliform gastritis, which has a heterogenous histology, including a group of diseases such as helicobacters gastritis and chemical gastritis (bile reflux).
Menetrier’s disease is a rare disease marked by a set of clinical, endoscopic and radiological symptoms. Certain histological criteria and, in particular, the intraepithelial lymphocytic infiltrate suggest a relationship between this disease and lymphocytic gastritis.
As regards gastric Crohn’s disease, the endoscopic lesions are restricted to the antral region mimicking antral rather than diffuse varioliform gastritis. The diagnosis is most often suggested by the existence of concomitant ileocolic lesions. Histologically, the only discriminating element is the granuloma formed by giant and epithelioid cells.

Las gastropatias hiperplásicas se presentan como modificaciones del relieve mucoso en forma de pliegues gigantes, cerebroides o por la asociación pliegues hipertróficos-nódulos-erosiones.
Pueden corresponder bien a afecciones neoplásicas, como la linitis plástica, el linfoma, bien a afecciones inflamatorias o degenerativas. En el presente trabajo tratamos del campo de las gastropatias hiperplásicas no tumorales. Los autores, basados en su experiencia en la gastritis linfocitaria y en sus estudios sobre la enfermedad de Menetrier proponen una nueva clasificación de las gastritis hiperplásicas, en la que se precisan mejor las entidades histopatológicas que constituyen el substrato de tales afecciones. Las gastropatías hiperplásicas halladas más habitualmente son la gastritis varioliforme, la enfermedad de Menetrier y la enfermedad de Crohn.
El aspecto histológico de la gastritis varioliforme difusa o la de localizatión fúndica corresponde a la entidad descrita bajo el término de gastritis linfocitaria. Se diferencia netamente de la gastritis varioliforme antral cuya base histológico es heterogénea y comprende un conjunto de afecciones diversas tales como la gastritis por Helicobacter, y la gastritis química (reflujo biliar).
La enfermedad de Menetrier es una enfermedad rara, caracterizada por unconjunto de sintomas clínicos, endoscópicos y radiológicos ; ciertos criterios histológicos, en especial el infilitrado linfocitario intraepitelial, sugieren una relatión entre ésta enfermedad y la gastritis linfocitartaria.
En cuanto a la enfermedad de Crohn de localizatión gástrica, hemos de señnalar que las lesiones endoscópicos son de exceptional localizatión fúndica o difusas y se identifican más con la gastritis varioliforme antral. El diagnostico se basa en el conocimiento previo de lesiones ileocolicas ; desde un punto de vista histólogico el único elemento discriminante es el granuloma de células gigantes y epiteiloides, excluido el microgranuloma.
