Les lymphocytes intraépithéliaux en pathologie gastrique inflammatoire

Intraepithelial lymphocytes in inflammatory gastric pathology

Résumé

En revoyant une série de gastrites chroniques ayant servi de contrôle à la maladie de Crohn gastrointestinale, les auteurs individualisent une image histopathologique particuliere, caractérisée par une infiltration massive de lymphocytes dans l’épithélium de surface et des cryptes. La fréquence de cette gastrite lymphocytaire a été estimée à partir de deux séries de biopsies endoscopiques. Elle est respectivement de 3,8 et 4,8 % dans les biopsies reçues entre 1980–1983 et 1984–1985. L’âge moyen des patients est de 54 ans et est identique à celui des autres types de gastrite chronique. Par contre, le rapport H/F qui est de 1,7 dans la gastrite chronique non spécifique tombe à 0,8 dans les gastrites lymphocytaires. L’image histologique de la gastrite lymphocytaire paraît suffisamment caractéristique pour proposer d’en faire une nouvelle entité anatomopathologique dont l’importance en clinique devra être précisée ultérieurement.

Summary

Reviewing a series of gastritis as control of gastroduodenal Crohn’s disease, the authors describe a peculiar histopathological picture characterized by a massive infiltration of lymphocytes inside the surface and pit epithelium. The frequency of this lymphocytic gastritis is estimated in two series of biopsies received between 1980–1983 and 1984–1985. The average age of the patients is 54 years and is identical to that of the other types of chronic gastritis. On the opposite, the sex ratio is 1.7 for chronic non specific gastritis and only 0.8 for lymphocytic gastritis. Lymphocytic gastritis appear sufficiently distinct from the histopathological point of view to consider it as an anatomopathological entity, the importance of which has to be specified in practical gastroenterology.
