L’intégration de la laparoscopie, de l’échographie et de la biopsie guidée sous échographie dans le diagnostic des tumeurs hépatiques bénignes

Laparoscopy integrates ultrasound and ultrasound guided biopsy for diagnosis of benign liver tumors

Résumé

Sur une série consécutive de 15 125 patients soumis à un examen ultrasonographique, 603 ont subi également une laparoscopie. Dans cette série, 311 tumeurs bénignes du foie ont été objectivées : 299 mises en évidence par l’ultrasonographie et 12 découvertes par hasard par la laparoscopie. A l’exception de 244 cas d’hémangiome typique (diagnostic ultrasonographique), 55 autres patients ont, en plus de l’échographie, subi d’autres techniques d’imagerie et/ou une biopsie à l’aiguille fine échoguidée. La laparoscopie a été utilisée dans 10 cas sur 55 ; chez 3 patients, son rôle a été fondamental, démontrant : 1 hyperplasie focale nodulaire, 1 hamartome biliaire, 1 stéatose multinodulaire.

Summary

We studied a consecutive series of 15,125 patients who underwent ultrasound examination ; 603 of them underwent laparoscopy also. We found 311 cases of benign hepatic tumors : 299 cases were detected by ultrasonography and 12, incidentally, by laparoscopy. Apart from 244 cases of typical hemangioma (diagnosed by US itself), in the other 55 cases US was integrated by other imaging techniques and/or US guided FNB. Laparoscopy was employed in 10 of these 55 cases ; in 3 patients (a case of focal nodular hyperplasia ; a case of biliary hamartoma ; a case of multinodular steatosis) it was the main diagnostic test.
