Intérêt de la ponction guidée percutanée à l’aiguille fine dans le diagnostic du cancer du pancréas

The role of thin needle percutaneous guided puncture in the diagnosis of cancer of the pancreas

Résumé

Entre avril 1979 et avril 1981, une ponction guidée percutanée à l’aiguille fine a été réalisée chez 90 patients (60 hommes et 30 femmes) chez qui une pathologie pancréatique établie avec certitude a pu être secondairement vérifiée (pancréatite ou cancer). Toutes les ponctions ont été réalisées par le même opérateur, toutes les lames ont été lues par le même cytologiste. Le mode de guidage le plus fréquemment utilisé fut l’échographie. En regroupant les résultats obtenus avec toutes les méthodes de guidage et en y incluant les résultats de certains examens jugés ininterprétables pour des raisons techniques, on obtient un taux de sensibilité de 77 % et un taux de spécificité de 100 % . Deux incidents sont survenus aux premiers temps de ce travail (deux perforations vésiculaires) traités chirurgicalement avec succès. Conduite par un opérateur entraîné collaborant avec un cytologiste averti, cette méthode simple et peu onéreuse apparaît efficace et fiable pour établir le diagnostic de cancer du pancréas avant toute décision opératoire éventuelle.

Summary

Thin needle percutaneous guided puncture was done on 90 patients (60 men, 30 women) between april 1979 and april 1981. In these patients, a pancreatic pathology was confirmed (pancreatitis or cancer). All the punctures were done by the same operator ; all the sections were examined by the same cytologist. Echography was the guidance method most frequently used. By classifying the results obtained with all the guidance methods and including certain examination results which were considered to be uninterpretable for technical reasons, the rate of sensitivity was 77 % and the rate of specificity was 100 % . Two incidents occurred in the beginning of this study (two perforations of the gallbladder) which were successfully treated by surgery. If done by a well trained operator in collaboration with an experienced cytologist, this simple and cheap method is effective and reliable in establishing the diagnosis of cancer of the pancreas before any surgical decision is taken.
